Tekrarlayıcı taş hastalığı olanlarda metabolik değerlendirme sonuçları

Özet
Kocaeli'nde tekrarlayıcı taş hastalığının metabolik nedenlerini araştırmak amacı ile kliniğimize müracaat eden tekrarlayıcı taş hastalığı olan olguların 24 saatlik idrar örneklerinin metabolik değerlendirmesi planlandı. Tekrarlayıcı taş hastalığı olan 80 olgu birinci grup ve anamnezinde taş hikayesi olmayan 26 olgu ikinci grup olarak çalışmaya dahil edildi. Sonucu etkileyebilecek tıbbı tedavi almakta olanlar, ileri derecede hidronefrozu olanlar, taşı olanlar, malinitesi veya anemnezinde bağırsak operasyonu olanlar çalışmaya dahil edilmedi. Tüm hastaların direkt üriner sistem grafileri, ve ultrasonik tetkikleri rutin olarak yapıldı. Predispozan anotomik bir patolojiyi ekarte etmek için intravenöz ürografiler değerlendirildi. 24 saat süreyle toplanan idrar örnekleri hacim, dansite, kalsiyum, ürik asit, sitrat, oksalat yönünden değerlendirildi. İdrar örnekleri aynı günde iki ayrı zaman diliminde (08-24 ve 24- 08) toplatıldı. İlk 16 saatlik örnek, içerisinde 30 cc 6 mol /lt HCl içeren kabın içerisine toplandı. İkinci örnek idrar, içerisinde 30 cc 0.3 mol/lt sodyum azide içeren kapta toplandı. Metabolik değerlendirme için sınır değerler olarak hiperkalsiüri için ♂;> 250 mg/g, ♀;> 200 mg/g, hiperürikozüri için >600 mg/g, hypositratüri için > 320 mg/g, hiperoksalüri için >45 mg/g alındı. Çalışmaya alınan olguların 69’u erkek, 37 si kadındı. Gruplardaki erkek /kadın oranı sırası ile 1. grupta 1.75, 2. grupta 2.25’ di. Hastaların 24 saatlik idrar analizinde hiperkalsiüri 15 (%14) olguda, hipositratüri 26 (%24.5) olguda, hiperoksalüri 13 (%12) olguda, hiperürikozüri 24 (%23) olguda tespit edildi. Yirmi dört saatlik idrar volümü çalışma grubunda daha fazla bulundu (1805.56±837.54cc, kontrol grubunda: 1109.17±186.13 cc). Multivariate analiz ve Fisher x2 testi ile sadece hipositratüri anlamlı bir etiyolojik neden olarak tespit edildi (p: 0.048). Sonuç olarak ilimizdeki tekrarlayıcı taş hastalığı olan olgularda hipositratüri temel etiyolojik faktör olarak bulundu. Hiperürikozüride ikinci sıklıkta görülen ama istatistiksel olarak anlamlı olmayan bir metabolik faktör olarak tespit edildi.
Abstract
Introduction: Kidney stone formation is a multifactorial and dynamic process and can be caused by many different and varied disturbances, some of which are poorly understood. Chemical composition of the urine, crystallization inhibitors and promoters, urine outflow conditions and urinary tract infections are main well known factors. In order to assess the metabolic causes of recurrent urolithiasis in our city, we evaluated the metabolic analysis of urine collected in 24 hours in patients with recurrent urolithiasis. Materials and Methods: 106 cases have been included in the study. 80 cases with a history of recurrent urolithiasis agreed as first group and 26 cases without a history of urolithiasis agreed as second group. The patients who were under medical therapy, which may affect the evaluation, were excluded. Also the patients who had hydronephrosis, current nephrolithiasis, malignancy and history of any bowel operation were not included in the study. All the patients underwent routine examination with kidney-ureter-bladder (KUB) graphs and ultrasonografic (USG) detection. Intravenous urograms (IVP) were evaluated to rule out predisposing anatomical pathology. 24 hours urine sample analysis has been performed for volume, density, calcium, uric acid, citrate and oxalate levels for all patients. The urine was collected in two different time periods in the same day 08-24 and 24-08. The first 16 hours sample was collected in a bottle containing 30 cc of 6 mol /L Hcl. The second 8 hours urine sample was collected in a bottle containing 30 cc of 0.3 mol/L sodium aside. The cut off points agreed for metabolic disorders were: Hypercalciuria for man >250 mg/d, for woman >200 mg/d, hyperuricosuria > 600 mg/d, hypocitraturia > 320 mg/d, hyperoxaluria > 45 mg/d. Results: The majority of the patients who were included in the study were male (69 vs. 37). The male/ female ratios in the groups were 1.75 and 2.25 (Group 1 and group 2 respectively). The mean age of the patients was 39.6 year (range; 17-59) in group 1 and 43.2 (range; 19-62) in group 2. The 24 hours urine analysis of the patients revealed hypercalciurea in 15 (14%) cases (13 in first and 2 in second group), hypocitraturia in 26 (24.5%) cases (22 in first and 4 in second group), hyperoxaluria in 13 (%12) cases (9 in first and 4 in second group), hyperuricosuria in 24 (%23) cases (17 in first and 7 in second group). The mean 24 hours urine volume was higher in the study group than control group (1805.56±837.54 versus 1109.17± 186.13 cc). Mean value for urine density was 1019±5.66 and 1021±3.95 (group 1 and group 2 respectively). Multivariate analysis and Fisher X2 test showed that hypocitraturia was the only significant etiological factor. Conclusion: Urine samples collection should be in standard practice settings. The critical aspect is preventing crystallization during collection. We used EUA guideline 2001 for urine collection. In our city hypocitraturia has been found as the major etiological factor for the patients with recurrent urolithiasis. Hyperuricosuria was found secondly common but not statistically significant predisposing metabolic factor for recurrent urolithiasis.
