Transüretral prostat rezeksiyonu uygulanan olgularda hipertansiyon varlığı kanamayı etkiler mi?

Özet

Bu çalışmanın amacı semptomatik benin prostat hiperplazisi (BPH) nedeniyle transüretral prostat rezeksiyonu (TURP) yapılan olgularda hipertansiyon varlığının kanama üzerine etkisi olup olmadığını araştırmaktır. Ocak.2000 ve Aralık 2002 tarihleri arasında, semptomatik BPH nedeniyle TURP uygulanan 91 olgu çalışmaya alınarak 25'i hipertansif, 66'sı normotansif olarak 2 gruba ayrıldı. Operasyon öncesinde olguların hepsinde yaş, uluslar arası prostat semptom skoru (IPSS), prostat hacmi ve maksimum idrar akım hızı (Qmax) değerlendirmesi yapıldı. Tüm olgularda operasyon öncesi ve sonrasında eritrosit, hematokrit ve hemoglobin konsantrasyonlarını içeren kan parametreleri kaydedildi. Normotansif ve hipertansif olgularda, operasyon süresi, kateterizasyon süresi ve TURP sonrası kan parametrelerindeki değişimler karşılaştırıldı. Operasyon öncesi değerlerde normotansif ve hipertansif hastalar arasında fark izlenmedi (p>0.05). TURP sonrası hem normotansif hem de hipertansif olgularda kan parametrelerinde düşme görüldü (pO.OOl). Normotansif ve hipertansif olgularda operasyon süresi, kateterizasyon süresi ve TURP sonrası kan parametrelerindeki değişimler yönünden fark izlenmedi (p>0.05). Çalışmamızın sonuçlan, semptomatik BPH nedeniyle TURP uygulanacak hipertansif olguların kan basınçları kontrol altına alındığında, normotansif hastalara göre kanamayla ilgili ek morbiditeye sahip olmayacaklarını göstermektedir.

Abstract

Introduction: The aim of this study was to investigate whether there was an effect of hypertension on haemorrhage in patients with symptomatic benign prostatic hyperplasia (BPH) undergoing transurethral resection of the prostate (TURP). Materials and Methods: Between January 2000 and December 2002, 91 patients with symptomatic BPH undergoing TURP were enrolled in the study, and the patients were divided into two groups; concomitant hypertension (n=25) and normotension (n=66). All patients were assessed at baseline using age, International Prostate Symptom Score (IPSS), prostate volume and maximum urine flow rate (Qmax). Pre and postoperative blood tests including red blood cells (RBC), haematocrit and haemoglobin concentrations were recorded in all patients. Operating time, duration of chateterization and the changes of the blood parameters after TURP were compared between the normotensive and hypertensive patients. Results: There were no significantly differences in baseline characteristics between the normotensive and hypertensive patients (p>0.05). Blood parameters significantly decreased after TURP in the both normotansive and hypertensive patients (p<0.001). There were no significantly differences in operating time, duration of chateterization and the postoperative changes of the blood parameters between the normotensive and hypertensive patients (p>0.05, for each parameter). Conclusion: Our study demonstrated that when compared with normotensive patients, hypertensive patients with symptomatic BPH undergoing TURP does not have additional morbidity related to blood loss when the blood pressure was undercontrol.
