Bir eğitim kliniğinde bile TUR-P altın standart mı?

Özet
TUR-P evrensel olarak prostat operasyonları arasında altın standart olarak kabul edilmektedir. Bu çalışmada birkaç uzman tarafından ve asistan eğitiminin bir parçası olarak yapıldığında TUR-P'nin, etkinliğinin ve güvenilirliğinin devam edip etmediğim araştırmayı amaçladık. Kliniğimizde 1992-1995 yılları arasında BPH nedeniyle TUR-P yapılan 48 hasta ortalama 8,8 yıllık takip sonucunda tekrar değerlendirilmiştir. Ayrıntılı anamnez, uluslararası prostat semptom skoru (I-PSS) ile skorlama, parmakla rektal muayene, PSA, üroflowmetri ile bütün hastalar araştırılmış, prostat boyutu ölçümü Transrektal Ultrasonografi (TRTJS) ile yapılmış, postnuksiyonel rezidü değerlendirilmesinde abdominal USG kullanılmıştır. Yaklaşık 9 yıllık uzun takip döneminde, hastalanıl %14,6'smda re-TUR-P gerektiği saptanmıştır. Ayrıca hastalardan 12'sinde (%25) üretra darlığı bulunmuştur. Bir hasta 5 yıl sonra gelişen prostat kanseri nedeniyle bu yönden değerlendirilmiştir. TUR-P'nin uzun dönem sonuçlan göz önüne alındığında, bir eğitim kliniğinde değişik cerrahlar ve uzmanlık eğitimi alan asistanlar tarafından uygulanmasına rağmen BPH'nın cerrahi tedavisinde düşük reoperasyon oranlan ile halen en iyi tedavi seçeneği olduğu ve standart asistan eğitimi içinde mutlaka bulunması gerektiği görüşüne varılmıştır.
Abstract
Introduction: TUR-P is universally accepted as the 'gold standard' among surgical options for BPH. In this study, conducted as part of a residency training program under the guidance of consultant urologists, we questioned hether the TUR-P could still be accepted as an effective and reliable technique. Patients and methods: In the period from 1992 to 1995, a total of 48 patients suffering from BPH were treated with TURP in our clinic and these patients were invited for reevaluation after a mean period of 8.8 years. In this reevaluation the patients were subjected to detailed history, international prostate symptom score (I-PSS), digital rectal examination, PSA and uroflowmetry. The size of the prostate was measured using transrectal ultrasonography (TRUS) and abdominal USG was used for the detection of postvoiding residual volume. Results: During approximate follow-up period of 9 years, 14,6% of the patients underwent re-TURP and, furthermore, 25% of them suffered from urcthral stricture. One patient developed prostate cancer at the fifth year following TURP and evaluated accordingly. Conclusion: Taking into consideration the long-term results of TURP, it is concluded that TURP is the best surgical treatment method for BPH, despite the fact that it was carried out by various surgeons and residents who are in urology training program in a teaching hospital. It is furthermore concluded that TUR-P must be included in standard residency training program.
