Prostat kanserinin histopatolojik ölçütleri ile neoanjiyojenezis ilişkisinin incelenmesi

Özet
Prostat karsinomu (PK) erkeklerde en sık görülen kanser türüdür. Tedavi için başlıca seçenekler sadece hormonal ablasyon, radyoterapi ve radikal prostatektomidir. PK’nin erken saptanması Prostat spesifik antijen (PSA) ölçümleri ile artmıştır. TNM evresi, Gleason grade ve serum PSA’sı gibi PK’nın gidişi için kullanılan prognostik faktörler başarı ile kullanılmaktadır. Ancak klinik, serolojik ve radyolojik değerlendirmelere rağmen hastaların %30’unda radikal prostatektomide (RP) vezikula seminalis ve kapsül tutulumu saptanır. Proliferatif aktivite, BCL- 2’nin ekspresyonu, p53 inaktivasyonu, neoanjiyojenezis gibi yeni prognostik faktörler ek bilgiler sağlar. Prostat karsinomunun tedavisi ile ilişkili histolojik özellikler ile ilgili çok az kullanılabilir veri vardır. Anjiyojenezis yani yeni damar oluşumunun prostat karsinomunda önemli prognostik bilgiler sağlayacağı düşünülmektedir. Bu çalışmanın amacı RP’de histopatolojik özellikler ve neoanjiyojenezis arasındaki ilişkiyi araştırmaktır. 1996-2002 yılları arasında lenfadenektomi ve RP yapılan 40 PK olgusu incelendi. Olgular, ameliyat öncesi serum PSA düzeyleri, yaş, Gleason’a göre ameliyat öncesi histoloji dereceleri ile RP’de diğer histopatolojik ölçütler (rezeksiyon cerrahi sınırları, ekstraprostatik yayım, lenf nodu metastazı, vezikula seminalis tutulumu) ve mikrovasküler dansite açısından sınıflandı. Olgular serum PSA düzeyleri ≤4 ng/ml, 4.1-10, 10.1-20, 20’nin üstü olarak gruplandı. Tüm prostat biyopsileri tekrar değerlendirildi ve her örnekte Glason skor, karsinom içeren alanın % değeri ve perinöral invazyon araştırıldı. RP’de mesane boynu ve distal üretradan tıraşlama ile alınan örneklerde ve çini ile boyalı diğer alanlarda tümör görüldüğünde pozitif cerrahi sınır olarak kabul edildi. Vezikula seminalis tutulumu bir veya iki taraflı kas tabakası infiltrasyonu olarak tanımlandı. Periprostatik yumuşak doku invazyonu ekstraprostatik yayım olarak tanımlandı. Lenf nodu tutulumu pelvik lenf nodlarında tümör varlığı olarak tanımlandı. Hematoksilen-eozin boyalı preparatlar (iğne biyopsi ve RP) değerlendirildi ve tümörler primer-sekonder Gleason skorlarına göre derecelendirildi. Her RP’den tek tanımlayıcı blok seçildi ve 3 mikron kalınlığında kesitlere CD 34 immun boyası uygulandı. CD 34 bir miyeloid progenitor hücre antijenidir ve tüm endotelyal hücrelerde saptanabilir. CD 34 monoklonal antikoru arter ve venlerin endoteli ile reaksiyon gösterir ve kapiller endotelinde çok yoğun bulunur. Mikrovasküler dansite sayımı CD 34 antijeni kullanılarak yapıldı. İmmun boyama yapılan tüm örnekler herhangi bir klinikopatolojik bilgi olmaksızın değerlendirildi. İmmunreaktivite x400 büyütmede değerlendirildi ve 10 yoğun damarlı alan sayıldı. Komşu mikrodamarlardan, tümör hücrelerinden ve bağ doku elemanlarından açıkça ayrı pozitif boyanan herhangi bir hücre veya hücre kümesi tek, sayılabilir damar olarak kabul edildi. Mikrodamar yoğunluğu 10 büyük büyütme alanının ortalaması olarak kaydedildi. İstatistiksel analiz için Pearson korelasyon, ki- kare ve Mann-Whitney U testleri kullanıldı. Ortalama yaş 65.5 (57-73) idi. Serum PSA değerleri 2-58 arasında bulundu. Mikrovasküler dansite ortalaması en az 28, en fazla 180 sayıldı. Mikrovasküler dansite ile ekstraprostatik yayım ve seminal veziküllerin tutulumu arasında istatistiksel olarak anlamlı ilişki saptandı. RP’de Mikrovasküler dansite ve Glason skorları arasında anlamlı ilişki saptanmadı. Anjiyojenezis tümör büyümesi ve matastazı için esastır ve bazı kanserlerde tümör saldırganlığının bir göstergesi olarak önerilmektedir. Anjiyojenik faktörler tümör hücreleri, yangı hücreleri ve stromal hücreler tarafından salınır. PK’de mikrovasküler dansitenin (MVD) değerlendirilmesi cerrahi sınırların ve veziküla seminalislerin tutulumu hakkında öngörü sağlayabilir. MVD’nin pozitif prediktif değeri ve bunun ameliyat öncesi ve sonrası ölçütlerle ilişkisine dair başka çalışmalar evreleme prosedürlerine kullanılabilir bilgiler sağlayabilir. Daha geniş serili çalışmalar özel hastaların tedavisinin etkinliğine dair bilgiler sağlayacaktır.
Abstract
Introduction: Prostate cancer is the most commonly diagnosed cancer among men in world. Primary options for treatment include observation only, hormonal ablation, radiation therapy, and radical prostatectomy. Prostate carcinoma detection has been increased with the use of prostate specific antigen. Successful attempts were made to establish useful prognostic factors for the outcome of prostate carcinoma, like TNM stage, Gleason’s grade and serum PSA level. However despite current clinical, serologic and radiologic evaluation of patients, as many as %30 of patients, are found to have disease that has penetrated the capsule and/or seminal vesicles on final pathologic analysis after radical prostatectomy. Various new prognostic factors that have been suggested to provide additional prognostic information include proliferative activity, overexpression of the apoptosis-inhibiting oncogene Bcl-2, neoangiogenesis and inactivation of the P53 tumor suppressor gene. Few data are available on the histological characteristics relating to the treatment of prostate carcinoma. Angiogenesis, the formation of new blood vessels, has been suggested to provide important prognostic information in prostate cancer. The goal of this study was to evaluate of histopathologic parameters and neoangiogenesis in men undergoing radical prostatectomy. Materials and Methods: We analyzed between 1996 and 2002, 40 consecutive patients with prostate cancer who underwent lymphadenectomy and radical prostatectomy. The patients who qualified for the study had a preoperative serum PSA level, age, preoperative histologic grade according to the Gleason histological scoring system and the other prognostic histopathologic parameters (surgical margin of resection, extraprostatic extent of cancer, lymph node metastases and seminal vesicle involvement) and microvascular density (MVD) in RP. Cases were categorized into groups of s-PSA≤4 ng/ml, 4.1-10, 10.1-20 and greater than 20. All prostate biopsies were reviewed and Gleason score, percent cores positive for carcinoma, and perineural invasion were recorded for each specimen. Positive surgical margins were those showing tumor in the shave bladder neck tissues, in distal urethral shave tissues, or in the inked edges of the remaining portions of the RP. Seminal vesicle invasion was defined as infiltration of the muscular coat of one or both seminal vesicle. Invasion of the perirostatic soft tissue was defined extraprostatic extention. Lymph node involvement, if the pelvic lymph nodes were involved with cancer. The hematoxylin-and-eosin- stained slides (needle biopsy and radical prostatectomy) were examined, and the tumors were graded by assigning both a primary and secondary Gleason score. A single representative block from each prostatectomy was selected and 3 micron thick sections were used for the CD 34 immunostaining. CD 34, a myeloid progenitor cell antigen also present in endothelial cells, is detectable in all types of endothelium. The monoclonal antibody to CD 34 reacts with endothelium of arteries and venules, and has been found to stain more intensely with capillary endothelium. Microvascularity density quantification was performed using CD 34 antigen immunohistochemistry as previously defined. All slides were evaluated for immunostaining in a blinded fashion without any clinicopathologic data. The immunoreactivity was evaluated at 400x magnification and 10 most vascular areas were assessed. Any positive cell or cell cluster, clearly seperate from adjacent microvessels, tumour cells and other connective tissue elements, was considered a single, countable microvessel. Microvessel density was then recorded as the mean of 10 high power fields. We used the Pearson correlation, test, chi-square test and Mann-Whitney U test for statistic analysis. Results: The mean age was 65.5 years (range, 57-73). Serum PSA ranged from 2-58 ng/ml. The mean Microvessel density for CD 34 ranged from a minimum of 28 to a maximum of 180. We were able to demonsrate a high degree of correlation between microvascularity in radical prostatectomy and that in the corresponding involvement seminal vesicle and ekstraprostatic extention. MVD was not significantly associated with Gleason grade in RP specimens in this study. Conclusion: Angiogenesis is essential for tumor growth and metastasis and has been proposed as a marker for tumor aggressiveness in several cancers. Angiogenic factors can be secreted by tumor cells, inflammatory cells and stromal cells. Histological evaluation of MVD in prostatic adenocarcinomas may improve the prediction of involvement of seminal vesicle and surgical margins. Additional data defining the positive predictive value of MVD and its relationship to other preoperative and postoperative parameters can provide a useful addition to our staging procedures. Obviously a larger series of cases may provide information that could help predicting the effectiveness of therapy in a particular patient.
