Tekrarlayan ön ve arka üretra darlıklarının tedavisinde Benique dilatasyon etkinliğinin internal üretrotomi ile karşılaştırılması

Özet
Bu çalışmadaki amacımız hem ön hem de arka üretra darlıklarında Benique dilatasyonun etkinliğini, sadece internal üretrotomi yapılan hastalarla karşılaştırmak ve tekrarlama riski yüksek olan darlıklarında cerrahi gereksinimi en aza indirmektir. Üretra darlığı nedeniyle cerrahi işlem uygulanan 39 erkek hasta prospektif olarak değerlendirildi. Hastalar 3 gruba ayrıldı. Grup I (n=12) ön üretral darlığı olanlar ve internal üretrotomiden sonra dilatasyon protokolüne alınanlar, grup II (n=17) arka üretral darlığı olanlar ve internal üretrotomiden sonra dilatasyon protokolüne alınanlar, grup III (n=10) ise sadece internal üretrotomi yapıldıktan sonra, darlık nüksettiğinde tekrar internal üretrotomi yapılan hastaları oluşturmaktaydı. Ortalama 15.1.1±2.9 aylık takip sonucunda grup I'de %25, grup II'de %11.7 ve Grup III'de %60 darlık nüksü görüldü. Hastalarda ameliyat öncesi maksimal akım hızları grup I'de 6.1±2.3 mL/s, grup II'de 5.3±2.5 mL/s, grup III'de 5.9±2.5 mL/s (p>0.05) bulunurken ameliyat sonrası son takipte sırasıyla 13.2±2.8 (p 0.05) mL/s olarak tespit edildi. Tekrarlayan üretral dilatasyonun, kontrol grubu ile karşılaştırıldığında arka üretral darlıklarda daha fazla olmak üzere tüm olgularda nüksleri önlemede oldukça başarılı bir yöntem olduğunu ve cerrahiye gereksinimi azalttığını düşünüyoruz.
Abstract
Introduction: Urethral stricture disease is still a major problem in men. Although excellent results are obtained with surgical reconstruction recurrent urethral stricture development is very often during long-term follow-up after open surgery and endoscopic procedures. The objective of our study is to compare the efficacy of Benique dilation in both anterior and posterior urethral strictures with patients who underwent direct visual internal urethrotomy (DVIU) and to minimize the need for surgery in urethral strictures in which the recurrence risk is high. Materials and Methods: A total of 39 male patients who underwent surgical procedures for urethral stricture were evaluated prospectively. Patients whose stricture was shorter than 2 cm underwent endoscopic procedures while the patients whose stricture was longer than 2 cm and/or complete underwent open surgery. Patients were divided into 3 groups: Group I (n=12), patients who has anterior urethral stricture and taken to dilation protocol after DVIU, Group II (n=17), patients who has posterior urethral stricture and taken to dilation protocol after DVIU, Group III (n=10), patients who underwent DVIU again when having a recurrence after undergoing only DVIU. Results: Stricture recurrence rates in group I, II and III were 25%, 11.7% and 60%, respectively after a mean follow-up of 15.1±2.9 months. Preoperative maximal flow rates in group I, II and III were 6.1±2.3, 5.3±2.5 and 5.9±2.5 mL/s, respectively (p<0.05) while the postoperative maximal flow rates were 13.2±2.8 (p<0.05), 17.4±5.3 (p<0.05) and 9.4±2.8 8 (p>0.05) mL/s, respectively. Conclusions: Urethral dilation with Benique sounds is a simple, easy, cheap and well-tolerated procedure. We suggest that although it is much more successful in posterior urethral strictures when compared with control group; dilation protocol is a very successful method in preventing recurrences in all cases. It also reduces the need for open surgery.
