Pelvik ektopik böbrekte laparoskopi eşliğinde perkütan nefrolitotomi ile taş alınması

Özet
Pelvik ektopik böbrekte büyük taşları olan hastaların tedavisi komşu abdominal organlar ve vasküler yapılar nedeniyle halen zorluklar göstermektedir. 43 yaşında erkek hasta yaklaşık 5 aydır olan karın ağrısı ve makroskobik hematüri yakınması ile kliniğimize başvurdu. Hastanın öyküsünde özellik yoktu. Yapılan intravenöz ürografi, abdominal ultrasonografi ve trifazik tomografi tetkikleri sağ pelvik ektopik böbrekte, üst polde yerleşmiş yaklaşık 31 mm boyutunda taş olduğunu ortaya koydu. Genel anestezi altında litotomi pozisyonunda 6 Fr açık uçlu katater ektopik böbreğe retrograd olarak yerleştirildi. Umbilikus altına yerleştirilen 10 mm ve sağ ve sol iliak fossaya yerleştirilen 5 mm trokarlar eşliğinde periton arka yaprağı açıldı ve laparoskopik görüş altında ince iğne böbreğe yerleştirildi. İdrarın aspire edildiğinin görülmesinden sonra kılavuz kateter üst polden böbreğe yerleştirildi. Trakt dilatasyonu sırasında kılavuz telin tekrarlayan girimlere karşın çıkması nedeniyle böbrek üst polünde yaklaşık 15 mm insizyonla parankim açılarak 30 Fr nefroskop şaftı dilatatörler üzerinden böbreğe yerleştirildi. Daha sonra 24 Fr nefroskop böbreğe ilerletilerek taş görüldü. Taşın pnömatik litotriptör ile parçalanmasından sonra tutucu forsepsler ile taşlar alındı. İşlem sonunda 20 Fr nefrostomi tüpü böbreğe ve 26 Fr drende pelvise yerleştirildi. Hasta ameliyat sonrası sorunsuz bir dönemin ardından 6. gün taburcu edildi.
Abstract
Introduction: There are still some challenges in treatment of the patients with large urolithiasis in pelvic ectopic kidneys because of close abdominal visceral organs and vessels. A-43-years old male patient admitted to our clinic with complaint of abdominal pain and macroscopic hematuria lasting for 5 months. Past medical history of the patient was unremarkable. The intravenous urography, abdominal ultrasonography and triphasic computed tomography revealed a right ectopic pelvic kidney with a 31 mm stone in upper pole of the kidney. In lithotomy position under general anesthesia, a 6Fr open-ended ureteral catheter was introduced into the ectopic kidney, retrogradely. Through a 10 mm trocar inserted` under umbilus and 5 mm trocars inserted in the right and left iliac fossae, posterior peritonea was incised and under laparoscopic vision a needle was introduced into the kidney. After aspiration of urine from spinal needle a guide-wire was inserted. Because of dislocation of the guide-wire during tract dilatation from kidney in spite of repetitive attempts, an approximately 15 mm incision was performed to the upper pole of the kidney, and a 30 Fr sheath was introduced into the kidney over a renal dilatator. A 24 Fr nephroscope was inserted through the kidney and the stone was seen. After fragmentation of the stone with pneumatic lithotriptor, the stone fragments were removed out with grasping forceps. At the end of the procedure, a 20 Fr reentry nephrostomy tube was placed into the kidney and a 26 Fr drain was positioned into the pelvis. At postoperative day 6 patients were discharged.
