Stres üriner inkontinans tedavisinde fasyal sling ve TVT ameliyatlarının karşılaştırılması: erken istenmeyen yan etkiler ve sonuçlar: randomize klinik çalışma

Özet
Bu çalışmamızda stres üriner inkontinans nedeni ile faysal sling ve TVT ameliyatları yapılan hastaların ikinci yılın sonundaki başarılarının ve istenmeyen yan etkilerinin karşılaştırılmasını amaçladık. 2002-2005 yılları arasında stres üriner inkontinansı olan 70 bayan hasta çalışmaya alındı ve hastalar iki gruba randomize edilerek 1. gruba (34) faysal sling, 2.gruba (36) TVT ameliyatları uygulandı. Hastaların yaş grupları her iki grupta da 54±3 idi. Her iki grup arasında ortalama pariteleri, kiloları ve rektosel-sistosel derecelerinde anlamlı istatistiksel bir fark yoktu. Hastalara stres testi ve ürodinami tetkikleri ile tanıları kondu. Günlük kullandıkları ped sayıları 1. grupta 2,56±0,7, 2.grupta ise 2,44±0,8 adet idi. Ameliyat sonrası hastalar, 1. hafta, 4. hafta, 3. ay, 6. ay, 12. ay ve 24. ay kontrollere çağrıldılar. Başarı, stres aktivitelerinde tam kuruluk hali olarak kabul edildi. Her iki grup hastalarda ameliyat sonrası 1. haftadan sonra günlük kullandıkları ped sayısında azalma görüldü. 2. yılın sonunda 1. grupta ameliyat öncesi kullanılan ortalama ped sayısı 2,56±0,7 adet iken ameliyat sonrası hastaların %92’sinde tam kür sağlandı. 2.grupta ameliyat öncesi kullanılan ortalama ped sayısı 2,44±0, 8 adet iken ameliyat sonrası %94’inde tam kür sağlandı. İki grup arasında anlamlı fark ortalama taburculuk süreleri (2. grup 1,1±0,5 gün, 1. grupta 3,7±1 gün p=0,003), ortalama ameliyat süreleri (1. grupta 70±5 dakika, 2. grupta 34±4 dakika p=0,003), ortalama sondalı kalma sürelerinde (1. grupta 7,6±2,2 gün, 2. grupta 1.3±0,5 gün p=0,002) görüldü. Ameliyat sırasında istenmeyen yan etkiler olarak, üriner enfeksiyon 1. grupta %5,8 ikinci grupta %8,3 olarak görüldü, mesane perforasyonu 1. grupta %8,8, ikinci grupta %8,3 oranında görüldü, vajinal kanama 1. grupta %2,9, ikinci grupta görülmedi. Üriner retansiyon 1. grupta %11,7, ikinci grupta %5,5 oranında görüldü. Geç istenmeyen yan etki olarak 6. ayın sonunda urgency tarif eden hastalara ürodinami uygulandı. 1. grupta %11,7, ikinci grupta ise %8,3 oranında de novo urge saptandı. İstenmeyen yan etkiler açısından her iki grup arasında anlamlı bir farka rastlanmadı (p=0,03). Sonuç olarak TVT ve Fasyal sling işlemlerinin iki yıllık izlenimimizde başarı oranları ve istenmeyen yan etkileri arasında bir fark saptanmamıştır. Aralarındaki en büyük fark olarak TVT işleminin süresinin kısa olması, hastanın taburculuğunun ve sonda alınma süresinin daha kısa oluşudur.
Abstract
In this study we aimed a comparison of the achievements and complications at the end of the second year of the patients who had fascial sling and TVT operations due to stress urinary incontinence. Materials and Methods: 70 patients with stress urinary incontinence were studied between 2002 and 2005. They were randomly grouped into two, 36 patients underwent fascial sling operations (1st Group), and 34 patients underwent TVT operations (2nd Group). The mean age was 54±3 in both groups. There were no differences in the mean parity, weight, grade of rectocel-cyctocel. Their diagnoses were based on stress tests and “uro-dynamic” investigations. The numbers of the sanitary pads they consumed daily in the 1st group were 2.56±0.7, in the 2nd group were 2.44±0.8. Following the operations, the patients were routinely examined at 1 week, 4 weeks, 3 months, 6 months, 12 months and 24 months. The criterion of success was the condition of full dryness in the course of their stress activities. Results: In both groups, sanitary pads numbers were decreased in the first week after the operation. The average number of sanitary pads in the first group was 2.56±0.7 before the operation, 92% of the patients were fully cured at the 2nd year. Similarly, 94% of the patients in the second group were fully cured while their consumption of sanitary pads was an average of 2.44±0.8 before the operation. Meaningful differences were observed between the two groups in the following aspects: Their average hospitalization (1.1±0.5 days in the second group; 3.7±1 days in the first group; p=0.003); operation time (70±5 minutes in the first group; 34±4 minutes in the second group; p=0.003); catheterization time (7.6±2.2 days in the 1st group; 1.3±0.5 days in the 2nd group; p=0.002). Perioperatif complications were urinary tract infection in the 1st group 5.8% in the 2nd group 8.3%, bladder perforation in the 1st group 8.8% in the 2nd group 8.3%, vaginal bleeding in the 1st group 2.9% and it was not occurred in the second group, urinary retention in the 1st group 11.7% 5.5% in the 2nd group. The late complication was de novo urgency. Urodynamic study was performed the patients who suffered from urgency after 6 month. Urgency was observed 11.7% in the 1st group and 8.3% in the 2nd group. There was no difference between the two groups in terms of the occurrence of complications (p=0.03). Conclusion: In conclusion, a two-year observation of the TVT and Fascial Sling processes showed that there was no difference between the two processes in terms of their success rates and the complications involved. The most notable difference was that the process of TVT was shorter in duration just as the discharge time of the patients from the hospital and the removal of the catheter.
