Özet
Yaşlanma ile birlikte erkeklerde serum androjen düzeyleri azalmakta ve buna bağlı belirtiler oluşmaktadır. Bu nedenle hipogonadizmi olanlarda testosteron replasman tedavisi kullanımı artmaktadır. Bu tedavi ile gelişebilecek olası yan etkilerden bir tanesi artmış prostat kanseri riskidir. Birçok yazar androjenlerin mevcut prostat kanserini tetiklediği konusunda görüş birliği içinde olsalar da androjenlerin bu aşamadaki rolleri tam olarak net değildir. Hormon seviyeleri ve prostat kanseri arasında herhangi bir ilişki göstermek için yapılan çalışmalar belirsiz sonuçlar vermiştir. Yaşlı ve klinik olarak belirgin prostat kanseri olmayan erkeklerde testosteron replasman tedavisi ile prostat kanseri gelişiminin hızlanması önemli oranda şüphe ile karşılanmalıdır. Dikkatli takip, tedavi alan hastalarda önerilmekte ve iyi bir tıbbi seçenek oluşturmaktadır.
Abstract
With aging, androgen levels decrease in men and a result of this hypogonadism symptoms occur. Therefore testosterone supplementation to treat hypogonadism is increasing. One of the possible complication for this treatment is increased prostate cancer risk. Although most authors agree that androgens are involved in the exacerbation of existing carcinoma for the prostate, the role of the androgens in this process is not clear. Attempts to demonstrate a correlation between hormone levels and prostate cancer have yielded inconsistent results. It is highly doubtful that when testosterone therapy is performed at older men, any potential prostate cancer promotion effect will be clinically manifested in the absence of already established cancer. As with any therapeutic regimen, careful monitoring of the patient receiving treatment is recommended and constitutes good medical care.
