Böbrek transplantasyonu immünosüpresif tedavisinde siklosporin mikroemülsiyon formu ile tacrolimusun karşılaştırılması

Özet

Amaç: Böbrek transplantasyonu immünosüpresif tedavisinde siklosporin mikroemülsiyon formu ile tacro- limusun etkinlik ve yan etkilerinin karşılaştırılması Gereç ve yöntemler: Mart 2003 ile Haziran 2005 arasında böbrek nakli yapılan, başlangıç immünosüpre- sif tedavisi olarak basiliksimab, siklosporin mikroemülsiyon formu, mycophenolat mofetil ve prednizolon veya basiliksimab, tacrolimus, mikofenolat mofetil ve prednizolon verilen olgular dahil edildi. Olguların 18 aylık takip sonuçları değerlendirmeye alındı. Alıcılar taburculuklarından sonra ilk bir ay haftada bir, ikinci ay 15 günde bir, takip eden dönemde ayda bir poliklinikte kontrol edildi. Toplam 41 olgu çalışmaya alındı. Olgular demografik özellikler, akut rejeksiyon, kardiyovasküler ve metabolik yan etkiler, greft fonksiyonu, enfeksiyon, hirsutismus ve gingiva hiperplazisi, kozmetik yan etkiler, nefrotoksisite, hepatotoksisite, ilaç değişimi, sağkalım yönlerinden değerlendirildi. Bulgular: Olguların yaş, cins, donör tipi, diyaliz süreleri, preoperatif ve postoperatif sistolik kan basıncı, kreatinin, hepatotoksisite, nefrotoksisite, diyabetes mellitus gelişimi, enfeksiyon sıklığı arasında anlamlı fark bulunmadı. Hospitalizasyon süresi SiklosporinA grubunda daha uzun olarak saptandı. Akut rejeksi yon Tacrolimus grubunda 5 (%23.8), SiklosporinA grubunda ise 4 olguda (%20.0) görüldü. SiklosporinA grubunda Tacrolimus grubuna göre kolesterol ve trigliserid değerleri postoperatif dönmede anlamlı yükselme bulundu. SiklosporinA grubunda ilaç değişim nedeni olan kozmetik yan etkiler (hirsutismus, gingiva hiperplazisi) Tacrolimus grubunda hiç gözlenmedi. SiklosporinA grubunda toplam 8 olguda (%40) Tacrolimus grubunda ise sadece 1 olguda ilaç değişimine gidildi. Gruplar arasında exitus ile sonuçlanan vaka yoktu. Sadece SiklosporinA grubundan bir olgu greft kaybı olarak kabul edildi. Sonuç: Kozmetik yan etkiler ve hiperlipidemi için Tacrolimus, SiklosporinA’ya üstün bulunmuştur. Hiperlipideminin kardiyovasküler hastalık riski dikkate alındığında takrolimus daha tercih edilebilir görülmektedir. Ancak yinede immünosüpresif rejim bireysel değerlendirilmelidir.

Abstract

Objective: To compare the efficacy and side effects of a cyclosporine microemulsion and tacrolimus in kidney transplantation immunosuppressive therapy. Materials and methods: Between March 2003 and June 2005, the patients who had undergone kidney tx sur- gery and who were administered either basiliximab, a cyclosporine microemulsion, mycophenolate mofetil and prednisolone or basiliximab, tacrolimus, mycophenolate mofetil and prednisolone for base-line immuno- suppressive therapy were recruited to our study. We evaluated the results after an 18-month follow-up period. The donors were called back weekly for a follow-up in the first month, fortnightly in the second month and then monthly for 18 months after discharge. A total of 41 patients were included. The demographics, acute re- jection, cardiovascular and metabolic side effects, graft function, infections, hirsutism, gingival hyperplasia, cosmetic side effects, nephrotoxicity, drug changes and the survival of the patients were evaluated. Results: There were no significant differences among the patients with regard to age, sex, donor type, dialy- sis periods, preoperative and postoperative systolic blood pressures, creatinine levels, hepatotoxicity, neph- rotoxicity, the occurrence of diabetes mellitus and the incidence of infection. The duration of hospitalization was prolonged in the cyclosporin A group. Acute rejection emerged in 5 patients (23.8%) in the tacrolimus group and in 4 patients (20%) in the cyclosporin A group. In the cyclosporin A group, the cholesterol and triglyceride levels were significantly higher than the tacrolimus group. The cosmetic side effects (gingival hyperplasia and hirsutism) as a reason for a change in medication were only observed in the cyclosporin A group, not in the tacrolimus group. A medication change was made in 8 patients in the cyclosporin A group and in 1 patient in the tacrolimus group. No death was observed in either group. Graft loss was observed in only 1 patient in the cyclosporin A group. Conclusion: Regarding the cosmetic side effects and hyperlipidemia, tacrolimus was found to be superior to cyclosporine A. Where hyperlipidemia is considered to be a risk factor for cardiovascular disease, ta- crolimus use should be considered a more acceptable treatment course. However, the immunosuppressive regimen should be individually evaluated.
