Bir toplu sünnet uygulaması ve sonuçlarının medikolegal yönden değerlendirilmesi

Özet

Ülkemizde çocuklardaki sünnet işlemi, sıklıkla dinsel inanış ve bazen de tıbbi nedenlerle gerçekleştirilmektedir. Toplu sünnet törenleri ise sosyal yardım kurumlarının desteklediği yaygın bir uygulamadır. Bu çalışmada bir toplu sünnet girişimi ve gelişen komplikasyonlar değerlendirildi. Çocuklardaki bu uygulamanın etik yönü ve risklerini günümüz yasal düzenlemeleri eşliğinde değerlendirerek, sünnet işleminin standardizasyonu ve toplumun bilinçlenmesi gereğine dikkat çekmeyi amaçladık. Yerel yönetim tarafından düzenlenen ve 4 sağlık memurunun görev aldığı bir toplu sünnet organizasyonunda, girişimin denetimi ve işleme yönelik kayıtlar pratisyen hekim tarafından yapıldı. Girişim sonrası olguların kontrolü ve komplikasyonlann tedavisi referans hastanede üroloji uzmanları tarafından gerçekleştirildi. Yaş ortalaması 7.4±2.2 (3-11) olan toplam 122 çocuk, 280 dakikada (ort: 9.9 dakika/olgu) sünnet edildi. Erken ve geç dönemde toplam 23 (%19) olguda komplikasyon saptandı ve gerekli tedavileri yapıldı. İşlem öncesi onay veren ailelerin hiçbiri gelişen komplikasyonlar nedeniyle yasal işlem yapılması talebinde bulunmadı. Ülkemizdeki mevcut yasalar değerlendirildiğinde, sünnet işleminin uzman olmayan kişilerce bile gerçekleştirilebileceği ve gelişen komplikasyonlann cezai sorumluluğun düşük, tedavisinin ise hekimin sorumluluğunda olduğu belirlendi. Sonuç olarak hekim olmayan kişilerce sağlıksız ortamlarda gerçekleştirilen toplu sünnet uygulamaları, hekim kontrolünde bile olsa yüksek komplikasyon riski taşımaktadır. Hekim ve sivil toplum örgütlerince ailelerin bilinçlendirilmesi, işlemin standardizasyonunun yapılması, çocuk hakları ve tıbbi etik kurallar doğrultusunda günümüz koşullarına uygun yasaların uygulamaya konulması varolan sünnet karmaşasının çözümünde ana unsurlar gibi gözükmektedir.

Abstract

In Turkey, circumcision is generally performed due to religious and sometimes medical reasons while mass circumcisions are common organizations supported by social aid institutions. In this study, a mass circumcision and the complications of this organization were evaluated. We aimed to emphasize both the necessity for the standardization of the circumcision procedure and the need for the education of the community in terms of the risks and ethical perspectives of this organizations. In a mass circumcision organization supported by the local authority, four health technician were assigned. Â general practitioner designated as supervisor of the procedure was also accountable for examination and documentation. The Urologists of the reference hospital performed the control of the children and the treatment of any complications after the procedure. All children (n:122) with the mean age of 7.4±2.2 (range: 3-11) were circumcised in 280 minutes (mean: 9.9 min for each). In the early and late postoperative period, a total of 23 (19%) children received appropriate treatment for complications. None of the parents demanded for legal trial for these complications while they signed informed consent before. As a result, a mass circumcision performed by non-physicians in unhealthy environments carries a high risk potential even under control of a physician. For the solution of the confusion around this common procedure, there seems to be same measures for medical and civilian associations such as making the parents more sensible, standardization of the circumcision procedure accordingly and updating the related laws in favor of child-rights and medical ethics.
