Özet

Bu çalışmada üriner stres inkontinansın cerrahi tedavisinde modifiye pubovajinal sling operasyon tekniğimizin uzun dönem sonuçlarını irdelemeyi amaçladık. Stres inkontinans tarif eden hastalardan ileri derece sistosel, uterin prolaps ve norojenik inkontinansı olan hastalar dışlanarak prospektif çalışma planlandı. Tüm hastalardan detaylı hikaye alındı, litotomi pozisyonunda sistosel, rektosel, üretral hipermobiliteleri değerlendirildi Tüm hastalara ürodinamik tetkik yapıldı. Basan tam kuruluk olarak değerlendirildi. Sadece urge tarifiyle inkontinans tarif eden hastalar medikal tedaviyle kontinan kalıyorsa başanlı gruba dahil edildiler. 1996 senesinden itibaren 59 hasta çalışmaya dahil edildi. 15 hastada geçirilmiş başansız inkontinans cerrahi öyküsü vardı, 1 hasta multipl operasyon geçirmişti. 22 hasta 24 aylık takibi tamamladı. Uzun dönem takibi tamamlayan hastalann %86'sında basan saptandı. 9 hastada medikal tedaviye cevap veren urge inkontinans mevcuttu. 16 hasta (%27) üretral kateter 7. gün alındıktan sonra idrar yapamadı ve rekateterizasyon gerekti. Hiçbir hastada bu durum 4 haftayı geçmedi Denovo urge inkontinans gelişen 9 hastada (%15.4) bu hal kalıcı olup, antikolinerjik tedaviye yanıt verirlerken ilacın kesilmesi semptomların tekrarına neden oldu. Modifiye rektus pubovajinal sling operasyonu uzun dönem sonuçlan yüz güldürücü olup gerçek stres inkontinans cerrahi tedavisinde etkili bir yöntem olarak görülmektedir. Bununla beraber gelişebilecek ve kalıcı olabilecek urge inkontinans açısından hasta bilgilendirilmelidir.

Abstract

In this study, we aimed to evaluate the long term results of a modified rectus fascial pubovaginal sling procedure for the treatment of stress urinary incontinence. A prospective study was designed for evaluation of this procedure. Female patients with stress type urinary incontinence were included in the study. Patients with high grade cyctoceles, uterine prolapsus, and those with a history of neurogenic incontinence were excluded from the study. All patients had a thorough history and physical examination, special attention being paid to cystocele, rectocele, urethral hipermobility and stress test and urodynamics. The success was accepted as total eradication of stress urinary incontinence. Those with urge incontinence that respond to medical therapy were counted as successful. 59 patients fulfilled the inclusion criteria since 1996. 15 patients had previous antistress incontinence surgeries of which 1 had unsuccessful multiple procedure. 22 patients have completed the 24 months follow up period. The success rate of the operation was %86 in patients who completed the long term follow up. 9 patients reported urgency incontinence that responded to anticholinergic medical therapy. 16 patients (%27 whose urethral catheter was removed post operative 7th day were unable to void and recatheterization of less than 4 weeks resulted in resolvement of retention. No undo surgery was required for any complication. Denovo perminant urgency occurred in 9 patients ( %15.4) but they responded to medical therapy. In this group of urgency patients discontinuation of medication resulted in relapse of their urgency symptoms. Rectus fascial pubovaginal sling procedure is an effective and durable surgical alternative in the treatment of stress urinary incontinence. However the patients should be warned about the possibility of developing urgency symptoms that require and respond to medical therapy long term postoperatively.
