Özet
Enürezis nokturna püberte sonrası yaş grubunda %1 oranında görülen hastayı olduğu kadar yakın çevresini de etkileyen sosyal bir sorundur. Etiyolojide gösterilebilir organik bir rahatsızlık nadir olsa da, hasta ve yakınlarının geniş bir araştırma yapılması konusundaki beklentileri yadsınamaz. Eylül 2000-Mart 2001 tarihleri arasında Enürezis nokturnası olan ve yaşları 19-23 (Ortalama: 21) arasında değişen 40 hasta çalışma grubuna alındı. Sekonder enürezisli, diürnal enürezisli ve geçmişinde spinal, pelvik ve kafa travması geçirmiş olan hastalar çalışma dışı bırakıldı. Tüm hastalara ayrıntılı öykü, fizik muayene, idrar analizi ve kültürü, ürografi ( IVP ), rezidüel idrar tayini, açlık kan şekeri (AKŞ), antidiüretik hormon (ADH ), üretrosistoskopi, sistometri ve psikiyatrik muayene yapıldı. Bir hastada nörojen mesane, bir hastada anlamlı rezidüel idrar saptandı. Diğer otuz sekiz hastada herhangi bir organik patoloji saptanmadı. Psikiyatrik konsültasyonları sonucunda bir hastada psikotik bozukluk (Önceden tanı konmamış ve tedavi almamış), bir hastada borderline kişilik bozukluğu saptandı. Diğer 38 hastanın psikolojik profili normaldi. Püberte sonrası enüretik hastalarda anamnezin ve fizik muayenenin uygun ve titiz yapılması halinde basit bir idrar analizinin tetkik olarak yeterli olabildiği, ultrasonografinin rutine sokulduğu kliniklerde muayenede gözden kaçabilecek patolojilerin yakalanabildiği, invaziv ve noninvaziv olan diğer araştırma yöntemlerinin gerekli olmadığı sonucuna varıldı. Bu hastalarda uygun tedaviye hemen başlanılması gerektiği gösterildi.
Abstract
The prevalence of enuresis nocturna is approximately %1 at the post puberty age group and it socially affects the patients'' close relatives as well as the patient himself. Although there is rarely an organic disorder responsible for the etiology, the expectations of the patients and their relatives about a detailed research cannot be denied. Between September 2000 and March 2001,40 patients with enuresis nocturne, aged 19-23 (average age 21 years), were included in the research. Patients who had secondary enuresis, diurnal enuresis and pelvic, spinal or head trauma in the past were excluded. Physical and psychological examination, biochemical and microbiological urea analysis, urography (IVP), residual urea determination, blood glisemia analysis, antidiuretic hormone (ADH) analysis, urethrocystoscopy and cystometry were performed to all the patients. Neurogenic bladder and residual urea were determined in two different patients respectively. No pathological disorders were determined in the remaining 40 patients. During psychological consultations, pschotic disorder (previously undiagnosed and untreated) in one patient and borderline personality disorder in another patient were determined. Psychological profiles of the remaining 38 patients were normal. Provided that detailed anamnesis and physical examination are performed, an ordinary urea analysis would be sufficient for the investigation of patients with post puberty enuresis and any pathologies, that are missed in clinics were ultrasonographic analysis is routinely performed, can be detected. We also believe that there is no need for other invasive or noninvasive methods and the appropriate treatment should immediately be started for such patients.
