Özet

Selim prostat hiperplazisi (BPH) cerrahi tedavisinde standart transüretral elektrorezeksiyon (TURP) ile plazmakinetik vaporizasyonun (PKV) etkinlik ve güvenirliliğini prospektif randomize olarak karşılaştırmayı amaçladık. Mart 2001-Mart 2002 tarihleri arasında kliniğimize, selim prostat hiperplazisine bağlı alt üriner sistem şikayetleri ile başvuran toplam 45 hasta değerlendirmeye alındı. Hastaların prostat büyüklüğü $\leq$60 ml olup, malinite şüphesi olmayan gruptaydı. Hastalara 2:1 randomizasyon oranı ile 30 PKV, 15 TURP uygulandı. Hastalar ameliyat süresi, transfüzyon gerekliliği, ameliyat sonrası irigasyon ve kateterizasyon süreleri; preoperatif ve postoperatif 1. ve 3. ayda IPSS semptom skoru ve maksimum idrar akım hızları açısından karşılaştırıldı. Her iki gruptaki hastaların yaş ortalaması 63 (49-80) yıl olup, prostat volümleri 49±8 ml'dir. Ameliyat süresi PKV yapılanlarda 57.5±17 dk, TUR yapılanlarda 70.0±15.2 dk olup anlamlı fark vardır (p=0.008). Ameliyat sonrası irigasyon süreleri, TUR 29±10 st, PKV 12±10st olup PKV grubunda belirgin olarak düşüktü (p 0.05). TUR grubundan 2 hastaya transfüzyon gerekirken, PKV grubunda transfüzyona gerek duyulmadı. PKV tekniğinin en önemli avantajları; kısa operasyon, irigasyon ve kateterizasyon süreleri, minimal kanama, izotonik mayi kullanımı ve elektrik akımı kullanılmamasıdır. Sonuç olarak erken dönemdeki bu sonuçlar BPH nın cerrahi tedavisinde plazma kinetik vaporizasyonun minimal invaziv yöntemler arasında iyi bir tedavi alternatifi olabileceğini göstermekte; fakat bu bulguların uzun dönem sonuçlar ve daha geniş hasta grupları ile desteklenmesi gerekmektedir.

Abstract

A prospective study was conducted to evaluate efficacy and safety of PlasmakineticTM energy against standard transurethral resection of prostate (TURP) in the surgical management of BPH. Between March of 2001 and that of 2002, a total of 45 patients with a complaint of lower urinary tract symptoms admitted to our clinic, enrolled in this study with age ranging between 49 to 80 yrs (median 63). Randomization commenced with ratio of 2:1 (Plasmakinetic: TURP). Parameters which are operating time, blood transfusion, post-operative irrigation period and removal time of catheter were used in patients to compare. They were assessed at baseline and during the follow-up using the International Prostate Symptom Score (IPSS) and maximum urinary flow rate (Qmax). Mean prostatic volumes of patients were 49± 8 ml. The mean operation time of PKVP group (57.5±17 min) was significantly lower than that of TURP group (70.0 ±15.2 min) (p=0.008). Post-operative irrigation period was significantly shorter in PKVP group (mean 12±10 hrs) than that in TURP group (mean 29±10 hr) (p<0.001). Mean removal time of catheter was significantly longer in TURP (72±27 hr) than in PKVP (39±11) (p<0.001). Both groups had comparable mean IPSS and Qmax at baseline. They had significant improvement in these parameters after operation. In case of IPSS, especially at first month, PKVP group patients showed significantly better improvement (p<0.001). But there wasn't any Qmax difference between them (p>0.05). While blood transfusion was necessary for two patients in TURP group, not in PKVP group. Because of short operating time, less blood loss, shorter period of irrigation and catheterization, use of isotonic fluid and lack of electrical current, PKVP is safe and advantageous. Although these early period results have shown that this technique may be a good alternative between minimally invasive methods in the surgical management of BPH, studies with long term results and larger group of patients are essential
