Özet
Bu çalışmada üreteroselde değişik tanı ve tedavi yöntemlerinin etkinliğini değerlendirmeyi amaçladık. 1985-2000 yılları arasında kliniğimizde 40 hasta üreterosel tanısı alarak tedavi edildi. Uzun süre takibi yapılan 27 hastanın dosyaları retrospektif olarak incelendi. Ortalama takip süresi 1-15 yıl arasındadır. Tanıda intravenöz piyelografi (IVP), uretrosistografi, sistoskopi ve ultrasonografiden yararlanıldı. Böbrek fonksiyonunu tam değerlendirmek için bazı olgularda böbrek sintigrafisine başvuruldu. Böbrek fonksiyonu, vesiko-üreteral reflü, üreteroselin tip ve büyüklüğüne göre farklı cerrahi yöntemler uygulandı. Bu incelemelerle 11'i (% 40.7) tek sistemde basit üreterosel (grup 1), 11'i (%40.7) dupleks sistemde unilateral ve 5'i (%18.6) bilateral, ektopik üreterosel (grup 2) tesbit edildi. 11 basit ve 2 ektopik üreterosel intravezikal tipte idi. Kadın-erkek oranı 1.1:1 ve ortalama yaş 33 (1-65) olup en belirgin semptom üriner enfeksiyondu (% 62.9). Birinci gruptaki 7 hastaya endoskopik dekompresyon, 3' üne üreterosel eksizyonu ve reimplantasyon ve birine nefroüreterektomi ve üreteroselektomi yapıldı. Endoskopik dekompresyon yapılan 7 hastadan 6'sında (%85.7) başarılı sonuç alındı. Birinde yüksek grade'li vezikoureteral reflü geliştiğinden üreter reimplantasyonuna ihtiyaç duyuldu. İkinci grupta, üst ünitenin iyi fonksiyon yaptığı 9 hastadaki 11 üniteye aynı kılıf içinde Double-Barrel üreter reimplantasyonu ve üreteroselektomi yapıldı. 10'unda (%90.1) başarılı sonuç alındı. Diğer 7 olguda özelliklerine göre değişik ameliyat tipleri uygulandı. Endoskopik dekompresyon kolay ve non invaziv bir yöntem olup basit üreteroselin tedavisinde yüksek başarı oranı ile ilk seçenek olmalıdır. Geniş tek sistem basit üreteroselde üreterosel eksizyonu ve reimplantasyon alternatif bir metottur.Üst böbrek fonksiyonunun iyi olduğu çift toplayıcı sistemli ektopik üreteroselde, üreteroselektomi ve Double-Barrel reimplantasyonun en iyi cerrahi yöntem olduğu kanısındayız.
Abstract
The aim of our study was to evaluate different kinds of diagnostic procedures and treatment modalities of ureterocele. Between 1985 and 2000, 40 patients with ureterocele were diagnosed and treated in our department. The charts of 27 patients who had long term follow-up were reviewed retrospectively. The mean follow-up time was between 1 and 15 years. The diagnostic procedures included excretory urography (IVP), voiding cystouretrography, cystoscopy and ultrasonography. Renal scan was performed to obtain accurate renal function evaluation in some cases. Different kinds of surgical procedures were individualized based on renal function, presence of vesico ureteral reflux, the type and the size of ureterocele. This evaluation enabled the detection of 27 ureteroceles including 11 (%40.7) simple in single system (group 1), 11 (%40.7) unilateral ectopic and 5 (%18.6) bilateral ectopic ureterocele in duplex system (group 2). 11 simple and 2 ectopic ureteroceles were intravesical type. Male to female ratio was 1.1:1. The mean age of the patients was 33 years (1 to 65) and the most common presenting symptom was urinary infection (%62.9). In group I, endoscopic decompression was performed in 7, ureterocelectomy and reimplantation in 3, nephroureterectomy and ureterocelectomy in 1 patient. The results were succesfull in 6 patients (%85.7) with endoscopic decompression, but 1 patient needed ureteral reimplantation to prevent the side effect of reflux developed secondary to endoscopic incision. In group 2, Double-Barrel ureteral reimplantation in common sheath with excision of ureterocele was performed in 11 units of 9 patients with normally functioning upper renal unit. The success rate was (%90.1). In the remaining 7 patients different kinds of operations were performed according to their individual condition. We think that endoscopic decompression is an easy and non invasive technique and should be considered as the first choice of therapy in simple ureteroceles with high success rates. Ureterocelectomy and reimplantation is an alternative therapy for large size single system simple ureteroceles. Ureterocele excision and Double-Barrel reimplantation in common sheath is the best surgical modality for ectopic ureteroceles with functioning upper renal unit.
