Özet
Persitan Müllerian Kanal Sendromu (PMKS) nadir rastlanan bir erkek psödohermafroditizm çeşididir. Transvers testiküler ektopi, hemi uteri inguinalis ve bilateral epididimo testiküler fiizyon bozukluğunun eşlik ettiği, 46 xy karyotipli bir PMKS olgusu literatür verileri ışığında sunulmuştur. Literatürde 160 kadar vaka rapor edilmiş olmakla birlikte, epididimo-testiküler füzyon bozukluğunun eşlik ettiği başka bir PMKS olgusuna rastlamadık. Olgumuz epididimo-testiküler füzyon bozukluğu, hipoplazik epididim ve minimal düzeyde spermatogenetik aktivite gözlenmesi sebebiyle, bildirilen diğer vakalara ilave özellikler taşımaktadır. PMKS olgularında cerrahi yaklaşım, müllerian yapıların mümkünse korunması ve testislerin palpe edilebilir bir lokalizasyona getirilmesidir.
Abstract
Introduction: Persistent Müllerian Duct Syndrome (PMDS) is a rare form of male pseudohermaphroditism. A PMDS case with hernia uteri inguinalis, transverse testicular ectopia and fusional anomalies of the testis and epididymis in patient having normal 46 xy karyotype is presented in the light of relevant literature. Results: Although approximately 160 cases have been reported in the literature, we couldn't find another case with PMDS which fusional anomalies of the testis and epididymis accompanies to the persistence of Müllerian structures. Conclusion: Our case has some difference against the other cases which were reported in the literature with regard to epididymo-testicular fusion anomalies, epididymal hypoplasia, hipospermatogenesis. Basic principle of surgical intervention is to save the Müllerian structures and to bring the gonad to the palpable localisation.
