Çocuk üreter taşlarında ESWL tedavisinin etkinliği

The efficacy of ESWL for pediatric ureteral stones

Özet

Çocuk yaş grubu ureter taşlarında önemli bir tedavi seçeneği olan ESWL'nin bu olgu grubunda etkinliği ve yan etkileri araştırıldı. Temmuz 1992-Aralık 2002 tarihleri arasında pediyatrik yaş grubunda ($leq$16 yaş) 37 olguya üreter taşı nedeniyle ESWL uygulandı. Tüm olgular Siemens Lithostar litotriptör ile tedavi edildi. Preoperatif değerlendirmede tüm olgular tam idrar tahlili, idrar kültürü, kanama ve pıhtılaşma zamanları, direkt üriner sistem grafisi (DÜSG), intravenöz ürogram (İVP) ve ultrasonografi ile incelendi. İdrar kültüründe üreme olan olgular uygun antibiyotik baskısı altında tedavi edildi. Akciğer ve gonadlar için koruyucu kullanılmadı. Taştan arınma veya klinik önemsiz rezidüel fragmanlar (KÖRF) başarılı, 3 seansta fragmantasyon saptanmayan olgular ise başarısız kabul edildi. Sonuçlar ESWL tedavisinin son seansından 12 hafta sonra DÜSG ve İVP ile değerlendirildi. Ondört kız, 23 erkek çocuğun ortalama yaşı ortalama yaşı 9 yıl (1.5-16) olup taş lokalizasyonu 14 olguda üst, l olguda orta ve 22 olguda ise alt üreterde idi. Ortalama taş yükü.0.62 (0.15-1.76) cm2 olan olgulara ortalama 1.7 (1-8) seans ve ortalama 1779 (1000-3000) atış uygulandı. Bir olgu (%2.7) genel anestezi altında, geri kalan olgular sedoanaljezi ile tedavi edildi. Tedavi öncesi yardımcı yöntem olarak l olguya (%2.7) perkutan nefrostomi kateteri yerleştirildi. Tedavi sonrası 32 olgu (%86.5) taştan arınırken 3'ü (%8.1) KÖRF olmak üzere toplam basan %94.6 olarak belirlendi. Olguların 26'i (%70,8) tek seansta tedavi edildi. Başarısız olunan 2 olgu üreterorenoskopi ile tedavi edildi. KÖRF olan olguların l'inin 3 ay sonraki kontrolünde taşsız hale geldiği, diğer 2 olgunun stabil kaldığı görüldü. Bir olguda gelişen ve hastanede kalmayı gerektiren ateş dışında tedaviye bağlı komplikasyon görülmedi. Çocuk yaş grubu üreter taşlarında ESWL etkin ve güvenilir bir tedavi yöntemidir. Özellikle l cm2 ve altındaki taş boyutlarında tek seans ESWL ile yüksek başarı oranı sağlanabilmektedir.

Abstract

Introduction: We evaluated the success and complications of ureteral stones in children treated with ESWL. Material and Methods: Between July 1992-December 2002, 37 patients in pediatric age group were treated with ESWL .and analyzed. Urine analysis, urine culture, bleeding and clotting times and intravenous urogram was obtained routinely in the preoperative evaluation in all patients. Stone free and clinically insignificant residual fragments (CIRF) were defined successful and ESWL was regarded as failed if no stone fragmentation was seen after the 3rd session. Results: Mean patient age was 9 (1,5-16) years in 14 girls and 23 boys. 14 stones were located in the upper ureter, 1 and 22 stones were located in the mid and lower ureter, respectively. Patients were treated with Siemens Lithostar lithotriptor on outpatient settings. Patients with positive urine cultures were treated under appropriate antibiotics, starting at the day of ESWL. All patients were treated with sedoanalgesia except 1 (2,7%) who was treated under general anesthesia. Shielding of the lung fields or gonads was not used. One patient (2,7%) was catheterized by percutaneous nephrostomy catheter prior to ESWL. Patients with 0,62 (0,15-1,76) cm2 average stone load were treated for a mean number of 1,7 (1-8) sessions and an average of 1779 (1000-3000) shock waves. 32 (86,5%) patients were stonefree and CIRF were present in 3 (8,1%) after the treatments and total success rate was 94,6%. 26 patients (70,8%) were treated in one session. Ureterorenoscopy was performed in 2 patients regarded as failed. One of the 3 CIRF patients was stone free after 3 months and the 2 others were stabile. Fever that required hospitalization occurred in 1 patient and no other complication was seen. Conclusion: ESWL is safe and effective for appropriate ureteral stones regarding the size in pediatric population. Especially, at stone size equal or less than 1 cm2 with one session ESWL high success rates can be sustained.
