Özet
Psoas apsesi (PA) ender rastlanılan, genellikle tanısı zor ve geç konulan bir antitedir. Kendine özgün bir kliniğinin olmayışı ve özellikle sekonder vakalarda primer hastalığın bulguları ile karışması nedeniyle tanısı genellikle zor, geç ve hatta kimi zaman yanlış konmaktadır. Psoas kasının enfeksiyonu primer ve sekonder olarak meydana gelir. Sekonder enfeksiyon genellikle gastrointestinal sistem üriner sistem ve aksiyel iskelet sistemi ile ilişkilidir ve bu yapıların inflamatuar sürecine sekonder olarak ortaya çıkar Sekonder enfeksiyonlardan sıklıkla enterik bakteriler sorumludur. Primer psoas absesi ise nadiren görülür belirgin bir enfeksiyon kaynağı olmaksızın enfeksiyöz ajanın hematojen yolla psoas kasına gelmesiyle oluşur. En çok izole edilen ajan stafilokok aereustur (%88). Psoas apsesinde erken ve kesin tanı konarak mortalite ve morbiditenin düşürülmesi için ultrasonografı veya bilgisayarlı tomografinin uygun zamanda kullanılması gerekmektedir. Enfeksiyon etkeninin saptanması, uygun antibiyotik seçimi, açık veya perkutan olarak yapılan apse drenajı hastalığın prognozunu çok etkilemektedir. Burada erken tanının önemini vurgulamak amacıyla geç tanı konan ve bu nedenle komplikasyon gelişen bir psoas absesi olgusunu literatürü irdeleyerek sunuyoruz.
Abstract
Introduction: Psoas abscess (PA) is a rare identity, which is hard to diagnose. Since it has not a specific presentation, and especially for the secondary cases its signs can be confused with the primary disease; diagnosis is hard, delayed and sometimes incorrect. Infection of the psaos muscle can occur primarily or secondarily. Secondary infections usually take origin from inflammatory processes of gastrointestinal tract, urinary tract, or axial skeleton system. Frequently enteric bacteria are responsible for secondary infections. Primary psoas abscess is rarely seen. Infectious agent reaches to psoas muscle by the haematogenous way. Staphylococcus aereus is the most isolated agent (88%). Ultrasonography and Computerized Tomography should be used to appropriately identify and diagnose psoas abscess to reduce mortality and morbidity. Results: Prognosis is closely related to infectious agent, appropriate antibiotic usage, and abscess drainage either by open way or percutenous way. To show the importance of early diagnosis, here in we present a lately diagnosed, complicated psoas abscess in the scope of literature.
