Domuzlarda transvers retübülarize bağırsak segmenti (Monti tekniği) ile total üreter replasmanı uygulaması

Özet
Bu çalışmada domuzlarda transvers retübülarize bağırsak segmenti (TRBS) ile oluşturulan total ureter replasmanı sonuçları değerlendirildi.Yaş ortalaması 6 ay (5-7) ve ortalama ağırlıkları 25 kg (20-30) olan toplam 6 domuzda çift TRBS kullanılarak elde edilen tüp aracılığıyla sol ureter replasmanı gerçekleştirildi. Operasyonlar genel anestezi altında uygulandı. Tüm domuzlarda flank insizyon tercih edildi. Yaklaşık 2-2.5 cm'lik 2 adet ileum segmentinden elde edilen transvers retübülarize bağırsak segmentileri uç uca anastomoz edilerek 10-12 cm'lik ileal ureter meydana getirildi. Total üreterektomi sonrası total ureter replasmanı sol üreterlerde gerçekleştirildi ve sağ üreterler kontrol grubu olarak bırakıldı. Double j ureter kateterleri postoperatif 30-40. günlerde, suprapubik transvers insizyonla uygulanan ikinci bir operasyonla çıkarıldı. Operasyonlardan yaklaşık 2.5 ay sonra intravenöz piyelografıleri çekilen domuzlar sakrifıye edildi ve böbrekler, üreterler ve mesane bir bütün olarak çıkarıldı. Histolojik değerlendirme ışık ve elektron mikroskobisi yardımı ile gerçekleştirildi.Ortalama operasyon süresi 240 dak. (210-300) olup, 4 domuz çalışmayı tamamlayabildi Bir numaralı domuz ameliyat sonrası l. günde mezenter damarlarından kanama nedeniyle, diğer bir domuzda operasyon esnasında gelişen solunum durması nedeniyle kaybedildi. Diğer domuzların ortalama izlem süreleri 75 gün (70-80) olarak belirlendi. İki domuzda double J kateterler üretere migre olmaları nedeniyle çıkarılamadı. Ureter kateteri ile yapılan kateterizasyonda tümenlerin açık olduğu, ancak 2 ileal tüp arasındaki anastomoz yerinde darlık bulunduğu belirlendi.Bunun yanında hiçbir domuzda TRBS' nin mesane ve pelvis renalis ile olan anastomoz hatlarında darlık bulunmadığı saptandı. Postop intravenöz piyelografılerde 3 domuzda sol böbreklerin kontrast maddeyi belirgin olarak gecikmiş süzdüğü ve üreterlerin 1/2 üst yarısını içine alan grade 3-4 hidronefroz bulunduğu görüldü. Double j kateteri çıkarılamayan bir domuzun ise üreler kateterinin üreter dışına çıktığı belirlendi. Histolojik değerlendirmede 2 ileal tüp arasındaki anastomoz hattı ve etrafındaki bağ dokusunda darlıkla uyumlu olarak yer yer ileri ve orta derecelerde fibrozis gözlendi. TRBS tekniği kullanılarak gerçekleştirilen total üreter replasmanı, seğmenler orta üreter hasarlarında bir seçenek olmasına rağmen, bu çalışma sonuçlarına göre total üreter hasarlarında başarısız olarak gözükmektedir. Bunun nedeni iki tüp arasındaki ileoileal anastomoz yerlerinde gelişen şiddetli fibrozis olarak belirlenmiştir. Kesin yargıya varabilmek için bu konu ile ilgili hayvan çalışmalarının arttırılması gerekmektedir.
Abstract
Introduction: The aim of our study was to evaluate the results of a new technique named as transversely retubularised bowel segment (TRBS) for total ureteral replacement in pigs. Materials and Methods: We have performed left total ureteral replacement with 2 tubes which were formed by TRBS technique via flank incision in 6 pigs with a mean age of 6 months (5-7) and a mean weight of 25 kg (20-30). Pigs underwent the operations under general anestlıesia. TRBS, prepared using two ileum segments each 2-2,5 cm in length, were anastomosed end to end to get a 10-12 cm long tube. After urcterectomy, total ureteral replacement was performed in the left ureter and right ureter was used as the control group. Between the days of 30 and 40, double J catheters were taken out by a second operation via suprapubic transvers incision. Pigs were evaluated by excretory urography approximately 2.5 months after surgery and then sacrified thereafter, harvesting the kidneys, ureters and the bladders en bloc for macroscopic and histologic esamiııation by light and electron microscopes. Results: The mean operation time was 240 min. (210-300) and four pigs could end up in the study. One pig died due to the bleeding from one of the mesenteric arteries in postoperative fırst day and l pig died during the operation due to respiratory arrest. The double J catheters of 2 pigs couldn't be taken out because of catheter migration. Postoperative excretory urograms revealed that the excretion of the contrast material were delayed and there were grade 3-4 hydroureteronephrosis in the upper half of the ureter in three pigs. in the other pig, in which double J catheter couldn't be taken out, the catheter was out of the ureter. Catheterization displayed that the lumen vvere patent, however stenosis was detected only at the anastomosis region between the 2 ileal tubes. in the histologic evaluation, at the anastomosis sites between the two ileal tubes and in the surrounding connective tissue, moderate to marked fibrosis was seen consistent with stenosis. Conclusion: Although segmental ureteral replacement using TRIS technique vvas a choice in ureter injury, it was unsuccessful in total ureteral replacement according to the results in the current study. Marked fibrosis at the anastomosis sites between the tubes was accussed. in order to clarify this topic, further animal studies should be performed.
