Kronik skrotal ağrıda sol varikoselektominin önemi

Özet
Bu çalışmamızda kronik sol skrotal ağrı ve sol varikoseli bulunan hastaların, ağrıyla varikosel derecesi arasındaki ilişkisini ve operasyon sonrası ağrılarının geçip geçmediğinin araştırılmasını amaçladık. Kronik sol skrotal ağrısını varikosel dışında açıklayacak patolojisi olmayan hastalar Dubin ve Amelar’ın önerdiği şekilde derecelendirilip, 2 gruba ayrıldı. Hastaların skrotal ağrıları ameliyat öncesi ve sonrası 3. ayda Vizüel Analog Skala (VAS) kullanılarak değerlendirildi. Ameliyat öncesi ve sonrası bu 2 grup birbirleriyle karşılaştırıldı. Hastalara retroperitoneal ve inguinal yöntemlerle sol varikoselektomi uygulandı. Çalışmaya 34’ü Grade-2, 68’i grade-3 sol varikoseli olan toplam 102 hasta dahil edildi. Ameliyat öncesi grade-2 hastaların ortalama VAS değerleri 6.55±1.21, grade-3 hastaların ortalama VAS değerleri 6.72±1.20 olarak saptandı. hastaların ameliyat sonrası 3. aydaki ortalama VAS değerleri ise sırasıyla 2.11±1.38 ve 2.23±1.45 idi. Ameliyat öncesi ve sonrası grade-2’deki ve grade-3’deki hastaların skrotal ağrıları birbirleriyle karşılaştırıldığında, istatistiksel olarak anlamlı bulundu (p 0.05). Toplam 102 hastanın ameliyat sonrası 3. ayda yapılan kontrollerinde 87’sinde (%85.2) ağrının tamamen geçtiği,10’unda (%9.8) ağrılarda hafifleme olduğu ve inde (%4.9) ağrının şiddetinde değişme olmadığı görüldü. Kronik skrotal ağrısı olan olguların önemli bölümünde ağrı, varikoselektomi ile düzelir ve varikosele bağlı skrotal ağrı, varikoselin derecesinden bağımsızdır.
Abstract
Introduction: In this study we aimed to assess the presence of any correlation if exists between the left chronic scrotal pain and left varicocele grade and also between the left varicocelectomy and the relief of the pain after the procedure. Materials and Methods: Patients suffering from left chronic scrotal pain whom were diagnosed not to have any pathology other than varicocele for chronic scrotal pain were included to the study. Patients were graded according to the system proposed by Dubin and Amelar and they were divided into 2 groups; group 1: Patients with grade 2 varicocele and group 2: Patients with grade 3 varicocele. All patients were asked to fill 10 point Visual Analogue Scale (VAS) preoperatively and in 3rd postoperative month. All patients underwent left varicocelectomy with one of the retroperitoneal, inguinal, subinguinal and microscopic inguinal approaches. Results: 34 patients with grade 2, 68 patients with grade 3 left varicoceles (total 102 patients) were included to the study. Preoperative mean VAS scores were 6.55±1.21 in group 1 and 6.72±1.20 in group 2 respectively. Similarly, postoperative 3rd month mean VAS scores were 2.11±1.38 and 2.23±1.45 respectively. When preoperative VAS scores were compared to postoperative 3rd month VAS scores, a statistically significant difference was observed in both groups (p<0.05). Comparing both groups in terms of preoperative and postoperative 3rd month VAS scores there were no statistically significant difference between group 1 and 2 (p>0,05). In 87 patients of 102 patients (85.2%) pain relieved completely, in 10 patients (9.8%) pain relieved moderately and in 5 patients (4.9%) pain remained without any change. Conclusion: In majority of patients suffering from chronic left scrotal pain due to varicocele the pain is found to be independent from varicocele grade and it can be relieved with varicocelectomy.
