Özet

Alt üriner sistem semptomları (AÜSS) ile başvuran hastalarda serum ve tükürük serbest ve total prostat spesifik antijen PSA) düzeylerini araştırdık. Amacımız PSA'nın prostat bezine spesifik (özgü) olup olmadığını, tükürük PSA düzeylerinin AÜSS'li hastalarda bir tanı parametresi olup olmayacağını ortaya koymaktı. Alt üriner sistem semptomları olan 59 erkek hasta çalışma kapsamına alındı. Hastalardan aynı anda serum ve açlık tükürük serbest ve total PSA düzeyleri ölçüldü. Kulak burun boğaz (KBB) ve ürolojik muayeneleri yapıldıktan sonra rektal muayene ve transrektal ultrasonografıleri(TRUS) yapıldı.Hastaların KBB muayeneleri normaldi. AÜSS'li bu hastaların 11'inde başvuru anında üriner sistem enfeksiyonu mevcuttu. 45 hastaya benin prostat hiperplazisi (BPH), 8'ine kronik prostatitis, 6'sına nörojen mesane tanıları konuldu. Serum PSAMan yüksek çıkan veya TRUS'da şüpheli lezyonu olanlara TRUS eşliğinde biyopsi yapıldı.Biyopsi yapılan hastaların hiçbirisinde prostat kanseri saptanmadı. Yapılan istatistiksel analiz sonucu serum tPSA, sPSA ve s/tPSA oranlarının tükürükteki tPSA, sPSA ve s/tPSA oranlarından istatistiksel olarak anlamlı bir şekilde yüksek olduğu ortaya konuldu. Ayrıca, AÜSS ile başvuran bu hastaların BPH, kronik prostatitis ve nörojen mesane ayırımını sağlayacak serum ve tükürük PSA oranları saptanmadı.Prostata spesifik antijen (PSA), tükürük bezi gibi prostat dışı bezler tarafından da üretilmektedir. PSA, prostat kanserine spesifik bir tümör belirteci değildir. Bu nedenle prostata spesifik antijen olan adının güncelleştirilmesi gerektiğini düşünmekteyiz.

Abstract

Introduction: Prostate specifıc antigen (PSA) is the most useful tumor marker in urology. We investigated free and total PSA levels and free/total (f/t) ratio in the fasting saliva and compared them with serum levels in the patients presenting with lower urinary tract symptoms (LUTS). The objective of this study was to determine whether PSA is specifıc to prostate gland and saliva PSA levels are a diagnostic parameter in patients with LUTS. Materials and Methods: Serum and fasting saliva levels of free and total PSA were measured in 59 men with LUTS. The mean age of the patients was 57.6 years (range 39 to 73). Serum and fasting saliva samples were taken at the same time and studied on the same day with Immunoassay nıethod. Ear nose throat (ENT) and urologic examinations of patients were done by the specialists. We also applied digital rectal examination and transrectal ultrasonography. The patients who had high PSA levels ör evidence of prostate carcinoma at digital rectal examination and/or transrectal ultrasonography underwent transrectal ultrasonography-guided biopsy Results: There was no evidence of salivary gland disorder in these patients. Prostate cancer was not observed in any of the patients who underwent prostate biopsy. None of the 59 patients had renal functional impairment. Eleven of 59 (18.6%) men had urinary tract infection evidence. 45 of 59 (76.2%) patients had a diagnosis of benign prostat hyperplasia (BPH), 8 of 59 (103%) had chronic prostatitis and 6 of 59 (10.1%) had neurogenic bladder disease (diabetes mellitus, spinal cord injury and parkinson disease etc.). According to the results of the statistical analysis, serum levels of total PSA, free PSA and f/t ratio were significantly hjgher than saliva levels of total PSA, free PSA and f/t ratio. Determination of free and total PSA in saliva and serum was not useful for differential diagnosis in diseases such as BPH, prostatitis and neurogenic bladder disease in LUTS present patients. Conclusions: PSA is produced primarily by the epithelial cells of the ducts and acini of the prostate gland. in addition, PSA is produced by extra-prostatic glands, as well. PSA is not a cancer-specifıc tumor marker. Although the majority of serum levels of PSA was formed with expression of prostate gland, we believe that PSA is not specifıc to prostate cancer. Therefore, in our opinion, serum PSA does not lose anything from its importance in the diagnosing and monitoring of prostate cancer, but there is a necessity for a better terminology instead of PSA.
