Özet

Bu çalışmada TURP uygulanan hastalarda ameliyat sonrası erken ve geç dönemde değişen oransal PSA düzeylerinin incelenmesi amaçlanmıştır. BPH nedeni ile TURP uygulanan 41 hasta çalışmaya dahil edildi. Operasyon öncesi tüm olgulardan ayrıntılı anamnez alındı ve PRİ, IPSS, üroflovmetri ve serum PSA analizi uygulandı. TURP sonrası kalan prostat dokusu volümü başlangıç prostat dokusu volümüne bölünerek % kalan prostat volümü hesaplandı. TURP sonrası 6. hafta, 6. ve 12. ayda serum PSA düzeyleri yeniden ölçüldü. Bu değerlerin başlangıç PSA değerlerine bölümü ile de PSA değerlerindeki % azalmalar hesaplandı. TURP sonrası % kalan prostat volümü ile ameliyat sonrası 6. hafta, 6. ve 12. ayda tespit edilen % serum PSA düşüşü arasındaki ilişki ve TUR-P sonrası kalan % prostat dokusu miktarına göre olgular %50’den az ve fazla olanlar olarak iki gruba ayrılıp, PSA düşüşleri arasındaki fark değerlendirildi. p değerinin 0.05’ten küçük olması anlamlı kabul edildi. TURP ile rezeke edilen prostat volümü ortalama 43.1 gr, TURP sonrası 6. haftadaki ortalama serum PSA düzeyi 0.7 ng/ml, 12. aydaki ortalama serum PSA düzeyi ise 0.2 ng/ml olarak tespit edildi. Rezeksiyon sonrası % kalan prostat volümü ile 6. hafta, 6. ay ve 12 ay % PSA oranları arasında korelasyon saptanmadı (p>0.05). Olgular rezeksiyon sonrası % kalan prostat volümüne göre gruplandırıldıklarında, %50’den az prostat dokusu kalan 25 olgu ile, %50’den fazla prostat dokusu bırakılan 16 olgu arasında tüm takipler boyunca olan PSA düşüş oranlarında farklılık saptanmadı (p>0.05). TURP sonrası 6.hafta, 6. ay ve 12.ayda PSA düzeylerinin, sırası ile, operasyon öncesi değerlerin %31.8, %18.9 ve %17.8’ine düştüğü saptandı. TURP sonrası serum PSA düzeyi değişimlerinin bezin toplam volümünün ne kadarının çıkarıldığına değil, transizyonel zonun ne kadarının rezeke edildiğine bağlı olabileceğini düşünüyoruz. Sonuçlarımız TURP’li hastalarda 6.ay sonrasındaki serum PSA düzeyinin, operasyon öncesi değerin yaklaşık 20’sine ineceğini göstermektedir. Bu hasta grubunda gelişebilecek prostat kanserinin erken olarak saptanabilmesi açısından, değişen PSA dinamiğinin iyi yorumlanması gerekmektedir.

Abstract

Introduction: Prostate specific antigen originated prostatic epithelial cells. It is clear that its serum levels will change with prostatic interventions. In the previous studies that investigate this relationship, numerical changes on PSA levels and resected prostate volume were evaluated. We aimed to evaluate the changes on proportional PSA levels in early and late periods after transurethral prostatectomy. Materials and Methods: Forty-one patients who underwent TURP due to BPH (mean age was 68.3) were enrolled to the study. A detailed history, digital rectal examination, International Prostate Symptom Score, uroflowmetry and serum PSA analysis were performed to the all patients. The percentage of remaining prostate volume was calculated by the remaining prostate volume after TURP divided to the initial prostate volume. Serum PSA analysis repeated in the 6th week, 6th and 12th month, postoperatively. The percentage decrease of in the PSA levels calculated by these levels divided to the initial PSA levels. The correlation between the percentage of remaining prostate volume and the percentage decrease of in the PSA levels at the 6th week, 6th and 12th month was investigated. The subjects were categorized according to the percentage of remaining prostate volume as smaller and greater than 50 %. Then, their PSA decreasing ratios compared. p<0.05 was accepted as significant. Results: Mean prostate volume was 68±32.3 g and mean serum PSA level was 3.1±2.45 ng/ml, preoperatively. Mean resection volume was 43.1±27.7 g and mean serum PSA levels in the 6th week, 6th month and 12th month were 0.7±0.3, 0.4±0.3 and 0.2±.01 ng/ml, respectively. The mean PSA decrease to the each grams of resected tissue in the 6th week, 6th month and 12th month were 0.06±0.01, 0.07± 0.02 and 0.08±0.04 ng/ml, respectively. There was no correlation between the percentage of remaining prostate volume and the percentage of PSA levels in the 6th week, 6th month and 12th month (p>0.05). There was no significant difference between the 25 cases whose percentage of remaining prostate volume was smaller than 50% and the 16 cases with this ratio greater than 50%. The percentage of remaining prostate volume was 39.1% and the 6th week, 6th month and 12th month PSA ratios were 31.8%, 18.9% and 17.8%, respectively. Conclusions: We think that the changes on PSA levels after TURP are related with resected transitional zone volume, not with the total resected prostate volume. Our findings showed that serum PSA levels decreased to 20% of initial PSA level in the postoperative 6th month. It is essential that the changing PSA dynamics might be well defined in order to detect a probable prostate cancer in these patients.
