Özet

Kabakulak, genellikle okul çağı çocuklarım ve genç erişkinleri tutan akut viral bir enfeksiyon olup özellikle puberte sonrası olgularda daha ağır seyreden ve istenmeyen yan etkilere neden olabilen bir hastalıktır. Son yıllarda aşılama programlarının daha yaygın uygulanması sonrasında erişkin yaştaki olgulara daha sık rastlanmaktadır. Epididimorşit postpubertal erkekte en sık görülen tükürük bezi dışı tutulum olup, görülme sıklığı %5-%37 arasındadır ve olguların %16-%65'inde her iki testis de tutulmaktadır. Testislerin tutulması sonrasında atrofı görülme oranı %40-%70 arasındadır ve olguların %30-%87' sinde infertilite gelişmektedir. Son yıllarda kabakulak orşiti tedavisinde interferon uygulanması ile akut belirtilerde daha erken iyileşme ve en aza indirgenmiş atrofı ve infertilite oranlan rapor edilmektedir. İnfertilite etiyolojisinde hatırı sayılır bir orana (%5) sahip olan kabakulak orşitinin tedavisi ile ilgili daha etkili yöntemlerin geliştirilmesi gerekmektedir.

Abstract

Introduction: Mumps is an acute viral infection, which is generally encountered among school-age children and young adults. In postpubertal patients, its clinical course is more severe and may cause serious complications. Recently, with the widespread immunization programs during childhood, its prevalence has shifted towards adults. Epididymis and testis is the most common extraparotidal site for mumps infection in postpubertal males. Incidence of mumps orchitis ranges between 5% and 37% and both testes are involved in 16% to 65% of the cases. Rate of testicular atrophy following mumps orchitis is 40% to 70% and infertility ensues in 30% to 87% of the patients. Recently, interferon treatment for mumps orchitis has been reported to provide an earlier improvement in symptoms and decreased rate of atrophy and infertility. Further studies should be performed to improve the treatment of mumps orchitis, which has a considerable role in the etiology of infertility (5%).
