Değişik yerleşimli üreter taşlarının üreteroskopik tedavisinde farklı litotripsi yöntemlerinin karşılaştırılması

Özet
Çalışmamızda, klasik rijid üreteroskop ile 12 yıl içinde uyguladığımız üreteroskopik girişimlerin endikasyonlarını, başarısını, istenmeyen yan etkilerini kullanılan değişik litotripsi teknikleri ile karşılaştırılması yapıldı. Mayıs 1992-Şubat 2004 tarihleri arasında değişik yerleşimli toplam 1305 üreter taşına üreteroskopi uygulandı. Hastaların yaş ortalaması 37.8 (17-62) idi. Taşlardan 36'sı (%2.8) üst üreterde, 143'ü (%11) orta üreterde ve 1126'sı (%86.2) alt üreterde yerleşmişti. Taşların çapı radyolojik olarak ölçüldü. Taş çapı 433 (%33.2) hastada 5 mm veya daha küçük, 759 (%58.1) hastada 6-10 mm ve 113 (%8.7) hastada 10 mm'den büyük idi. Ortalama taş çapı 7.8 mm (3-18) idi. Taşların tedavisinde 284 (%21.8) olguda ultrasonik litotripsi, 125 (%9.6) olguda elektrohidrolik, 225'inde (%17.2) pulsed-dye lazer ve 671 olguda (%51.4) pnömatik litotripsi uygulandı. Değişik yöntemlerin kullanımı ile yapılan üreteroskopinin ortalama başarısı %87.8 olarak bulundu. Ultrasonik, elektrohidrolik, pulsed-dye lazer ve pnömatik litotripsinin ilk girişimdeki başarıları sırayla %79.6, %72, %86.7 ve %92.7 olarak bulundu. Taşların yerleşimine göre, tek bir seans sonrası taşsızlık oranı üst üreter taşları için %75.4, orta üreter taşları için %92.2 ve alt üreter taşları için %95.8 olarak bulundu. 4 mm'den büyük rezidüel taşlar için taşın yerleşimine göre ikinci bir endoskopik işlem veya ESWL uygulandı. 5 mm'den küçük ve 6-10 mm arasındaki taşların kırılma başarısı sırasıyla %94,3 ve %89,9 idi. 10 mm'den büyük taşlar %79,2 ile en düşük başarıya sahipti. En sık rastlanan istenmeyen yan etkiler ameliyat sonrası ateş, taş migrasyonu ve küçük mukozal lezyonlar idi. Üreteroskopi, üreter alt ve orta bölüm taşlarında en seçkin tedavidir. Her tekniğin üstünlükleri ve olumsuzlukları mevcuttur. İstenmeyen yan etkiler; taşın yeri ve büyüklüğü, uygulanan teknik ve cerrahın deneyimi ile ilgili idi. Değişik teknikler arasında pnömatik litotripsi yüksek başarı ve düşük morbiditesi ile ilk tercihimizdir.
Abstract
Introduction: Advances in endoscopy have greatly improved the success rates of ureteroscopic lithotripsy, regardless of stone localization. URS is the favored endourologic treatment for patients with ureteral stones. In this study, we retrospectively reviewed our 12-years experience of ureteroscopy with different techniques in the treatment of ureteral stones. Materials and Methods: Between May 1992 and February 2004, 1305 patients underwent ureteroscopic lithotripsy for treatment of ureteral stones. The mean age of the patients was 37.8 (range from 17 to 62) years. Thirty six (2.8%) stones were located in upper ureter (lumbal), 143 (11%) in mid ureter and 1126 (86.2%) in the lower ureter (pelvic). The diameters of the calculi were measured radiologically. The calculi diameter was 5 mm or smaller in 433 (33.2%) patients, 6-10 mm in 759 (58.1%) and greater than 10 mm in 113 (8.7%). Average calculi diameter was 7.8 mm (range from 3 to 18). Ureteroscopy was performed under general or local anesthesia with 10 F rigid ureteroscope. Sequential ureteroscopic dilatation with olive tip bougie dilatators was done routinely. A 4.8 F ureteric catheter was placed to prevent ureteral stricture and reduce the incidence of postoperative renal colic secondary to ureteral edema. Ultrasonic lithotripsy was used in 284 (21.8%) patients, electrohydraulic in 125 (9.6%), pulsed-dye laser in 225 (17.2%) and pneumatic lithotripsy in 671 (51.4%). Follow-up studies included a plain film 24 hours later and 6 weeks after operation and renal ultrasound scan on postoperative 6th week. Results: The mean success rate of different lithotripsy techniques was 87.8%. The success rates of ultrasonic, electrohydraulic, pulsed-dye laser and pneumatic lithotripsy were 79.6%, 72%, 86.7% and 92.7% respectively. Stone free rates were 75.4% for the upper ureter, 92.2% for the mid ureter and 95.8% for the lower ureter. A second ureteroscopic lithotripsy or ESWL was performed for residual calculi according to the localization of stone. The success rate of pneumatic lithotripsy for 5 mm or smaller stones and 6-10 mm was 94.3% and 89.9%, respectively. The calculi which were greater than 10 mm had the least success rate which was 79.2%. The most common complications were postoperative fever, stone migration and small mucosal lesions. Conclusion: Ureteroscopy is the best choice of therapy for the lower and mid ureteric calculi. Each technique has different advantages and disadvantages. Pneumatic lithotripsy is our first choice among different techniques with the highest success and lowest morbidity rates. The complications were related to the localization and diameter of the calculi, the technique used and the surgeon's experience.
