Prostat kanserinde mikrodamar yoğunluğu, Kİ-67 ve P53 ekspresyonu ile klinikopatolojik ölçütler ve hasta prognozu arasındaki ilişki

Özet
Bu çalışmanın amacı, organa sınırlı prostat kanseri nedeniyle radikal retropubik prostatektomi (RRP) uygulanan hastalarda, klinikopatolojik ölçütlerle beraber mikrodamar yoğunluğu (MDY), Ki-67 pozitifliği ve p53 mutasyon varlığının prognostik değerini araştırmaktır. Kliniğimizde, organa sınırlı prostat kanseri nedeniyle sadece RRP uygulanan hastalardan klinik ve patolojik verilerine ulaşılabilen 69 hastanın patoloji örnekleri retrospektif olarak değerlendirildi. x200 büyütmede her bir etken VIII, CD31, CD105 pozitif hücre kümelerinin en yoğun olduğu alan MDY sayımı için seçildi. p53 nükleer aktivitesi gösteren tümör hücreleri %5’den daha fazla ise pozitif olarak kabul edildi. Nükleer Ki-67 boyanma yüzdesi negatif, %1-10 arası ve %10’dan fazla olarak gruplandırıldı. Hastaların ortalama takip süresi 37 (17-106) aydı. Toplam 17 (%24,6) hastada takipleri sırasında progresyon saptandı. Progresyon gösteren hastaların patolojik evre dağılımı progresyon göstermeyenlere göre daha ileriydi (p=0,01). Transizyonel zon PSA dansitesinin (TZPSAD) progresyon göstermeyen hastalarda gösterenlere oranla istatistiksel olarak anlamlı derecede düşük değerlere sahip olduğu saptandı (p=0.01). Çok değişkenli Cox regresyon analizine göre patolojik evre, Gleason skoru ve TZPSAD’nin progresyonsuz sağkalım için bağımsız birer risk faktörü oldukları saptandı. Kaplan-Meier analizinde ise, yüksek patolojik evre (pT3) ve yüksek Gleason skorunun (GS8-10) hastalarda progresyona kadar geçen süreyi istatistiksel olarak anlamlı derecede kısalttığı gözlendi (p=0.003, p<0.001, sırasıyla). RRP sonrası progresyonun önceden belirlenmesinde MDY, Ki-67 boyanma yüzdesi ve p53 protein ekspresyonun, patolojik evre ve GS gibi prognostik belirteçlerle karşılaştırıldığında ek bir katkı sağlamadığı görülmüştür
Abstract
Introduction: The aim of the study is to investigated the prognostic value of microvessel density (MVD), Ki-67 staining intensity and existence of p53 mutation together with clinicopathologic parameters in patients who underwent radical retropubic prostatectomy (RRP) due to the localized prostate cancer. Materials and Methods: In our clinic, pathological specimens of 69 patients who had clinical and pathological data after RRP were retrospectively evaluated. The area with the greatest density of each of CD31, Factor VIII and CD105 positive cell clusters were chosen separately for counting MVD in X200 magnification. Greater than 5% of the tumor cells which demonstrated p53 nuclear reactivity were graded as positive. The percentage of nuclear Ki67 staining was grouped as negative, 1-10% and more than 10%. Results: Mean follow-up of the patients was 37 (17-106) months. Disease progression was determined in 17 (24.6%) patients during follow-up. Distribution of pathological stage in progressive patients was more advanced than non-progressive counterpart (p=0.01). Transitional zone PSA density (TZPSAD) of nonprogressive patients was statistically lower than progressive patients (p=0.01). Multivariate Cox regression analysis revealed pathologic stage, Gleason score (GS) and TZPSAD as independent predictors of progression free survival. Kaplan Meier analysis revealed that higher pathologic stage and GS significantly shortened the time to progression (p=0.003, p<0.001, respectively). Conclusion: MVD, percentage of Ki-67 staining and p53 protein expression do not add any predictive value in regard to progression after RRP compared with prognostic markers such as pathologic stage and GS.
