Varikoselli olgularda spermatik ve periferik vendeki oksidatif stresin değerlendirilmesi

Özet
Varikoselli hastalarda oksidatif stresin sperm disfonksiyonu oluşturmada önemli bir rol oynadığı öne sürülmektedir. Bu çalışmada infertilite veya klinik varikosel ile başvuran olgularda varikoselektomi öncesi spermatik ve periferik venöz kan örneklerindeki oksidatif stresin değerlendirilmesi amaçlanmıştır. Klinik değerlendirmede varikosel tespit edilen 18 hasta çalışmaya alındı. Hastaların en az 2 semen örneği içeren semen analizleri değerlendirildi. Ameliyat sırasında spermatik ven ayrılmadan önce, testiküler venöz dönüş tarafından 2 cc kan alındı. Ameliyat sırasında eşzamanlı periferik venöz kan örneği alındı. Spermatik venöz kan ve periferik ven kanı eritrositlerinde süperoksitdismutaz, glutatyon peroksidaz ve glutatyon redüktaz, plazmada malondialdehit ve total antioksidan kapasite ölçüldü. Ameliyat sonrası 1. ve 3. aylardaki kontrollerinde hasta klinik olarak değerlendirildi. Hastaların ortalama yaşı 24,5±4,2 (16-30) yıldı. Fiziksel inceleme ile tespit edilen varikosellerin grade ortalaması 2,2±0,6 bulundu. Spermatik ven eritrositlerinde süperoksitdismutaz, glutatyon peroksidaz ve glutatyon redüktaz, periferik ven eritrositlerine göre anlamlı olarak düşük tespit edildi (sırasıyla p=0,012, p<0,0001 ve p=0,006). Testiküler ven plazmasında malondialdehit, periferik ven plazmasına göre anlamlı olarak yüksekti (p<0,0001). Total antioksidan kapasite ise periferik ven plazmasında anlamlı olarak yüksek bulundu (p=0,01). Varikoselli hastalarda spermatik vende oksidatif stresin daha yüksek olduğu saptanmıştır. Bu bulgular varikosele sekonder oluşan oksidatif stresin varikosele bağlı infertilite patogenezinde bir rol oynayabileceğini düşündürtmektedir.
Abstract
The aim of this study was to determine the free oxygen radicals from peripheral and spermatic vein blood samples of the men with infertility or clinical varicocele before varicocelectomy. Materials and Methods: A total of 18 patients with varicocele were recruited in this study. Semen analyzes of patients were evaluated at least two times. Blood samples of 2 cc were drawn from the testicular side of the spermatic vein before ligation of varicocele veins. Synchronous blood samples were drawn from the corresponding peripheral vein as well. Superoxide dismutase (SOD), glutathione peroxidase and glutathione reductase levels were measured at spermatic and peripheral blood erythrocytes; malondialdehyde and total antioxidant capacity were measured. All patients were clinically evaluated in postoperative 1st and 3rd months, as well as control semen analyzes were done. Results: The mean age of patients was 24.5±4.2 (range:16-30 years) and the grade of varicoceles that was detected with physical examination was 2.2±0.6. At spermatic vein eryhtrocytes SOD, glutathione peroxidase and glutathione reductase levels were statistically lower than peripheral vein eryhtrocytes (p=0.012, p<0.0001 and p=0.006, respectively). Spermatic vein plasma malondialdehyde level was statistically higher than peripheral vein plasma (p<0.0001). Total antioxidant capacity was higher than the peripheral plasma (p=0.014). Conclusion: Oxidative stress is significantly higher in the spermatic vein of patients with varicocele. We think that oxidative stress is one of the factors that is involved in the varicocele associated sperm dysfunction.
