Özet

Ateroskleroz patogenezinde önemli rolleri olduğunu bildiğimiz, Endotelin (ET), Nitrik oksit (NO) ve Vasküler Endotelyal Büyüme etkeni (VEGE)'nin sıçan klitoral kavernozal doku, serum düzeylerini ve östrojen yerine koyma tedavisi ile değişimlerini ortaya koyarak, kadın cinsel işlevi üzerine östrojenin etki yollarını ve etki eden mediatörleri araştırmayı planladık. Toplam 36 sıçan 4 grupta çalışmaya alındı. Ooferektomi yapılmayan 1. gruptaki sıçanlara (n=6) ve ooferektomi yapılan 2. gruptaki (n=10) sıçanlara 3 ay boyunca subkütan östrojen yerine koyma tedavisi yapıldı. Grup 3'teki (n=10) sıçanlara sadece ooferektomi, grup 4'teki (n=10) sıçanlara sham ameliyatı uygulandı. 3. ay sonunda tüm sıçanlar östroz siklüsünde sakrifıye edildi. Sıçanların serumu ve klitoral-kavernozal dokuları alınarak saklandı. Serum ve klitoral-kavernozal dokuda ET-1 ve NO düzeyleri ile klitoral-kavernozal doku VEGE boyanması gruplar arasında karşılaştırıldı. Östrojen replasmanı yapılan grup 1 ve 2'de serum ET-1 artarken klitoral-kavernozal doku ET-1 düzeylerinde azalma görüldü (p 0.0125). Östrojen yerine koyma tedavisi ile klitoral-kavernozal doku VEGE boyanması gruplar arasında farklılık göstermedi (p>0.05). Cerrahi olarak oluşturulmuş menapozda; östrojen replasmanının ET-1, NO'nun serum ve doku düzeyleri arasında denge sağlayıp, aterosklerozdan koruyucu etki oluşturarak cinsel işlev patogenezinde rol aldığını düşünmekteyiz.

Abstract

Introduction: Given to the well defined roles of endothelin (ET), Nitric oxide (NO), and Vascular Endothelial Growth Factor (VEGF), we investigated the estrogen mediated pathway of female sexual function and effective mediators, determining their serum and rat clitoral cavernous tissue levels and changes with estrogen replacement. Materials and Methods: A total of 36 rats were included in the study in 4 groups. Subcutaneous estrogen replacement for three months was performed in the first group without oopherectomy (n=6) and second group with oopherectomy (n=10). Rats in group 3 (n=10) had only oopherectomy, group 4 (n=10) had sham operation. At the end of the third month, all rats were sacrified in estrous cycle. Sera and clitoral cavernous tissue of rats were sampled and stored. ETİ and NO levels in serum and clitoral cavernous tissue and VEGF staining in clitoral cavernous tissue were compared between groups. Results: In groups 1 and 2, received estrogen replacement, while serum ET levels increased clitoral cavernous tissue ET-1 levels decreased (p<0.0125). In estrogen replaced groups, while serum NO levels increased (p<0.0125), tissue NO levels were higher than oopherectomy group, but not statistically significant (p>0.0125). Estrogen replacement and clitoral cavernous tissue VEGF staining showed no difference between groups (p>0.05). Conclusion: Our results suggest that estrogen replacement provides a balance between serum and tissue levels of ET-1 and NO and causes a protective effect against atherosclerosis, thereby, plays a role in pathogenesis of sexual function.
