Özet

Yolk-sac tümörü primer olarak gonadlarda gelişen, oldukça malin, AFP yüksekliği ile karakterize, ender germ hücreli bir neoplazidir. Ekstragonadal yerleşimli germ hücreli tümörler erişkindeki bütün germ hücreli tümörlerin %2-5’ini oluştururlar. Genellikle mediasten ve retroperitonda yerleşirler. Retroperiton yerleşimli germ hücreli tümörlerin primer olarak ekstragonodal gelişim gösterip göstermediği ya da regrese olmuş bir testiküler tümörün metastazı olup olmadığı hala tartışmalıdır. 46 yaşında, ekstragonadal- retroperitoneal yerleşimli Yolk Sac tümör tanısı alan ve yapılan ilk tetkiklerinde skrotal palpasyonu olağan ve ultrasonogrofide iki taraflı testiste atrofi haricinde bulgusu olmayan ancak kemoterapi tedavisi sırasında yapılan kontrol ultrasanogrofisinde sol testisinde 12mm çapında tümör ile uyumlu lezyon izlenen erkek olguya radikal orşiyektomi uygulandı. Testisin histopatolojik incelemesi sonucu, regrese germ hücreli (burnt-out) tümör tanısı alan olgu yayınlar eşliğinde tartışılarak sunuldu.

Abstract

Introduction: Yolk sac tumor is a highly malignant, rare neoplasm that usually arises in gonads and is generally characterized by a high serum alfa-fetaprotein (AFP). Extragonadal germ cell neoplasms (EGGCT) have been estimated to represent 2-5% of all adult germ-cell neoplasms. They have been reported most commonly in mediastinal and retroperitoneal sites. These tumors are thought to arise from germ cells misplaced during embryogenesis. It is still contraversial whether EGGCT are of truly extragonadal origin or metastatic lesions from undetected or regressed testicular cancer. A 46-year old man was diagnosed as having a retroperitoneal EGGCT composed purely of Yolk-Sac tumor. On palpation bilateral testis was normal and ultrasonography demonstrated no abnormal finding except atrophy. During postoperative chemotherapy the control scrotal USG examination showed a 12 mm suspicious tumoral mass in the left testis. Radical orchiectomy was performed and regressed testicular (burnt-out) carcinoma was the histologic diagnosis. The case is discussed and the relevant literature in this regard is reviewed.
