Özet
Kasık fıtığı onarımı sonrası tam testis iskemisi oldukça nadir görülmektedir. Otuz iki yaşında erkek hasta, sağ kasıkta strangüle fıtık nedeniyle, Prolen yama kullanılarak yapılan herniorafi ameliyatından altı gün sonra, sağ testiste şişlik, ağrı ve skrotumda kızarıklık yakınmalarıyla başvurdu. Muayenesinde testis oldukça sert ve hareketsizdi; palpasyonda yoğun hassasiyet vardı. Doppler ultrasonografide, sağ kasık bölgesinde hematom ile uyumlu görünüm gözlendi. Sağ spermatik kordda yer yer kan akımları izlenirken, sağ testiste kanlanma saptanmadı. Ameliyatta testisin tümüyle iskemik olduğu görüldü. Kasık bölgesinden hematom boşaltıldı. Spermatik kord serbestleştirildi, skrotal orşiektomi yapıldı. Patoloji sonucu yaygın iskemik nekroz ve kanama gösteren testis dokusu olarak bildirildi.
Abstract
Total testicular ischemia following inguinal hernia repair is a rare condition. A 32-year-old male patient presented with complaints of swelling in the right testis, pain, and rash on the scrotum six days after right inguinal herniorrhaphy using Prolene mesh for strangulated hernia. On physical examination, the testis was firm and fixed and there was severe tenderness on palpation. Doppler ultrasonography showed findings consistent with a hematoma in the right inguinal region, interrupted blood flow in the right spermatic cord, and no blood flow in the right testis. At surgery, total testicular ischemia was encountered. The hematoma was removed, the spermatic cord was released, and scrotal orchiectomy was performed. Pathological examination of the testicular tissue showed diffuse ischemic necrosis and hemorrhage.
