Özet

Amaç: Bu retrospektif klinik çalışmada, erişkin yaş grubunda uygulanan hipospadias onarım sonuçları sunuldu. Gereç ve yöntem: Ocak 1996 ve Mart 2009 tarihleri arasında 112 hasta hipospadias nedeni ile opere edildi. Hastaların yaşları ortalama 22 (dağılım 16-27) olup, 80’i (%71.4) primer, 32’si (%28.6) 1-12 kez operasyon geçirmiş sekonder olgulardı. Sekonder olguların tamamı, primer olguların ise 62’si (%77) sünnetli idi. Uygulanan cerrahi yöntemler ve sayıları sırayla Tipu 37, Mathieu 28, ağız mukozası 26, kombine yöntemler 9, Onlay flap 8, Duckett’ın ada flebi 3, Thiersch 1 hastadır. Bulgular: Primer sünnetli ve sünnetsiz 80 olgunun 15’inde sadece üretral fistül (%18.7), 3’ünde fistülle birlikte meatal darlık (%3.7) saptandı. Sekonder olguların altısında lokal penis cildi kullanıldı ve bu olguların birinde fistül, birinde üretrada tamamen açılma görüldü. Ağız mukozası kullanılarak onarılan diğer 26 olgunun 10’unda (%38.4) fistül, 3’ünde fistülle birlikte meatal darlık (%11.5) gelişti. Sonuç: Erişkin hipospadias onarımında, lokal penil cildin olmadığı ya da yetmediği ve ancak serbest bir greft kullanılarak onarılmak zorunda kalınılan sekonder olgularda başarı oranları daha düşük olmaktadır.

Abstract

Objective: In this retrospective clinical trial, results of hypospadias repair in the adult age group was presented. Material and methods: A total of 112 patients were operated due to hypospadias between January 1996 and April 2009. The mean age of the patients was 22 (range 16-27) years. Of the 112 cases, 80 (71.4%) were primary and 32 (28.6%) were secondary cases that had undergone 1-12 operations. All of the secondary cases and 77% of the primary cases (n=62) were circumcised. The number of cases by methods used are as follows: Tipu 37 cases, Mathieu 28 cases, Buccal mucosa 26 cases, combined method 9 cases, Onlay flap 8 cases, Duckett’s island flap 3 cases, and Thiersch 1 case. Results: Of the 80 primary cases, with and without circumcision, urethral fistula developed only in 15 cases (18.7%) and meatal stricture combined with the fistula occurred in 3 cases (3.7%). Local penile skin was used in 6 of secondary cases, and of those cases, fistula occurred in one and complete opening in the urethra was observed in one. Of the 26 cases operated using buccal mucosa, fistula occurred in 10 cases (38.4%) and meatal stricture combined with the fistula developed in 3 cases (11.5%). Conclusion: For hypospadias repair in adults, the success rates may be lower for cases in which a free graft must be used due to an insufficient or nonexistent amount of local penile skin.
