Genel üroloji pratiğimizde androlojik olguların yeri

Özet
Amaç: Bu çalışma ile üroloji kliniğine başvuran hastalara konan tanıların analiz edilmesi ve androloji olgularının genel üroloji pratiğimizdeki yeri ve ağırlığının saptanması amaçlanmıştır. Gereç ve yöntem: Hastanemiz üroloji polikliniğine 1 Ocak 2007-31 Aralık 2008 tarihleri arasında başvuran hastalar retrospektif olarak değerlendirilerek Hastane Bilgi Yönetim Sistemi üzerinden ICD-10 tanı kodlarına göre toplam bakılan hasta sayısı ve konulan tanılar belirlendi. En çok konulan ilk on tanı belirlendi. Ayrıca androlojik tanıların sıklığı ve sayısı ve standart üroloji tanıları içindeki oranları değerlendirildi. Bulgular: İki yıl içerisinde toplam 37,796 hasta muayene edilmiştir. Bu hastaların 27,077’si (%71.63) erkek, 10,719’u (%28.6) kadındır. ICD-10 tanı kodlama sistemine göre 333 adet farklı tanı kodu kullanılmıştır. Benin prostat hiperplazi 14,724 hasta ile ilk sıradadır. Polikliniğimize başvuran hastalar arasında sadece androlojik bir tanı alanların yüzdesi %4.40’tır (1,665 hasta). Androloji olguları içinde toplam 523 hasta ile en çok konan varikosel tanısı, ürolojik vakalar içinde 10. sıradadır. Androlojik tanılar içinde 2. sıklıkta (428 hasta) tespit ettiğimiz impotans, genel ürolojik vakalar içinde 13. sırada yer almaktadır. Üçüncü sıklıktaki androlojik tanısı 403 hastayla infertilitedir ve genel ürolojik vakalar içinde 14. sırada yer almaktadır. Sonuç: Üroloji tanıları içinde %4.40 oranında yer alan androloji olgularına gri alanda olduğunu kabul ettiğimiz tanıları eklediğimizde, androlojik tanıların ağırlığı %46.02’ye çıkmaktadır. Tüm bu tanıların içinde benign prostat hiperplazisi önemli bir yer tutmaktadır.
Abstract
Objective: In this study, it was aimed to analyze the diagnoses of patients who applied to urology clinic and to assess the severity and contribution of the andrology cases in our general urological practice. Materials and methods: The patients who applied to the urology outpatient clinic of our hospital between January 1, 2007 and December 31, 2008 were retrospectively evaluated; total number of patients and diagnoses were determined according to the ICD-10 diagnosis index through the Hospital Data Management System. The most common first ten diagnoses were determined. Additionally, the frequency and number of andrological diagnoses and their ratios among the standard urological diagnoses were evaluated. Results: In two years, 37,796 patients were examined. Of these patients, 27,077 (71.63%) were male and 10,719 (28.36%) were female. According to the ICD-10 index, 333 different diagnosis codes were used. Benign prostate hyperplasia has taken the first place with 14,724 patients. The percentage of patients who attended to our outpatient clinic with only an andrological diagnosis was 4.40% (1,665 patients). Varicocele was the first among the andrology cases with 523 patients and the 10th among all urology cases. Second most common diagnosis among the andrology cases was impotency (428 patients) that was 13th among all urology cases. The third most common andrological diagnosis was infertility with 403 patients and it was 14th among all urology cases. Conclusion: When we add the cases in the gray area to the percentage of 4.40% as the andrology diagnoses, the rate of the andrological diagnoses extends to 46.02% of all urological cases. Benign prostatic hyperplasia has a significant place among these diagnoses.
