Özet
Amaç: Hipospadias cerrahisinde turnike kullanımının ameliyat başarısı üzerine olan etkisini belirlemeyi amaçladık. Gereç ve yöntem: Polikliniğimize distal hipospadias şikayeti ile başvuran, yaşları 1-9 yıl arasında değişen 59 olgu çalışmaya alındı. Hastalar iki gruba randomize edildi. Grup 1de (n=32) olguların tamamına Snodgrass yöntemi ile hipospadias onarımı yapıldı ve kanama kontrolü amacı ile turnike kullanıldı. Turnike 20 dakikada bir gevşetilerek penil kanlanmanın devamı sağlandı. Grup 2de (n=27) operasyon esnasında kanama kontrolü amacı ile turnike kullanılmadı. Bulgular: Hastaların yaş ortalaması 5.5±2.6 yıl idi. Altı olgu sünnetliyken, 21 olguda kordi mevcuttu. Her iki grup yaş, kordi varlığı ve sünnetli olup olmaması açısından benzerdi. Postoperatif takiplerde her iki grupta 3er fistül ve Grup 1de 2 olguda, Grup 2de 1 olguda meatal stenoz gözlendi. Her iki grup arasında fistül ve meatal stenoz oluşumu açısından istatistiksel olarak anlamlı fark tespit edilmedi. Sonuç: Distal hipospadiaslı olguların onarımında peroperatif kanamayı azaltarak cerrahi işlemi kolaylaştıran turnike uygulanmasının operasyonun başarısı üzerine herhangi bir etkisi gözlenmedi. Hipospadias cerrahisinde peroperatif turnike uygulanması, morbiditede artışa neden olmadan ve güvenle kullanılabilir.
Abstract
Objective: We investigated the effect of using a tourniquet during operation on the success of hypospadias surgery. Materials and methods: The study include 59 patients between 1 and 9 years old who were referred to our clinic with distal hypospadias. The patients were randomized into two groups. Patients in Group 1 (n=32) underwent distal hypospadias repair with the Snodgrass technique and a tourniquet was used to control bleeding. The tourniquet was loosened every 20 min to allow penile blood Á ow. No tourniquet was used during the operation to control bleeding for patients in Group 2 (n=27). Results: The mean age of the patients was 5.5±2.6 years. Both groups were similar in terms of age, presence of cordi, and circumcision status. Postoperatively, 3 patients developed a Àstula in each group, and 2 patients in Group 1 and 1 patient in Group 2 developed meatal stenosis. There was no signiÀcant difference between the two groups in terms of Àstula and meatal stenosis development. Conclusion: Tourniquet application facilitated distal hypospadias surgery by decreasing the perioperative bleeding and had no effect on the surgery outcome. Perioperative tourniquet application in distal hypospa- dias surgery does not increase the morbidity and can be used safely.
