Prostat iğne biyopsisinde prostat volümünün önemi

Özet
Amaç: Prostat kanserinin teşhisinde ultrason rehberliğinde prostat iğne biyopsisi altın standart yöntem ol- masına rağmen, biyopsi şeması ve tekrarlayan biyopsi sayısı günümüzde halen tartışmalıdır. Prostat volümü arttıkça kanser yakalama oranının azaldığı bilinmektedir. Bu nedenle kanser saptama oranını arttırmak için kor sayısının arttığı (lateral biyopsiler ve saturasyon biyopsileri) değişik protokoller oluşturulmuştur. Bizde bu çalışmada prostat volümünün kanser yakalama üzerine olan etkisini kendi hasta serimizde değerlendirdik. Gereç ve yöntemler: Kliniğimizde 2004 ve 2010 yılları arasında PSAsı gri zonda olup (2.5-10 ng/mL) 10 kor prostat biyopsisi alınan 992 hastanın verileri değerlendirildi. Parmakla rektal muayenede (PRM) kanser şüphesi olan, 6 ve 8 kor biyopsi alınan, verileri eksik olan 500 hasta çalışma dışı bırakıldı. Kanser saptanan (Grup 1) ve patolojisi benign gelen (Grup 2) hastaların yaşları, free /Total PSAları (f/TPSA), PSA dansitele- ri (PSAD), prostat volümleri karşılaştırıldı. İstatistiksel analiz için student-t testi kullanıldı. Bulgular: Biyopsi sonucuna göre 418 hastada kanser saptanmazken 74 hastanın sonucu Prostat Ca olarak geldi. Grup 1de ortalama yaş 65±8.6 iken, Grup 2de ortalama yaş 64±8.1 bulundu. Grup 1de ortalama prostat volümü 45±23 cc iken Grup 2de 58±26 cc bulundu (p= 0.0001). Sonuç: Literatürle uyumlu bir şekilde kanserli grupta prostat volümü anlamlı derecede düşüktü. Benzer şekilde f/T PSA oranı Grup 1de daha düşüktü. PSAD ise daha yüksekti. Bu da küçük prostatlarda kanser saptama olasılığının yüksek volümlü prostatlara göre daha olası olduğunu desteklemektedir.
Abstract
Objective: Although ultrasound-guided prostate needle biopsy is the gold standard method in the diagnosis of prostate cancer, biopsy schemes and the number of repeated biopsies are still controversial today. It is known that the rate of prostate cancer decreases with increasing prostate volume. In this study, we evaluated the ef- fects of prostate volume on cancer detection in our patient series. Material and methods: In our clinic, data for 992 patients who had gray zone PSA levels and whose 10 core biopsies were taken between 2004 and 2010 were evaluated. Among them, 500 patients from whom 6 to 8 biopsies were taken, whose information was lacking and who had suspicious cancer findings after digital rec- tal examination were excluded. Patient ages, free/total PSA levels (f/TPSA), PSA densities (PSAD), Gleason scores, and prostate volumes were compared for those patients in whom cancer was detected (Group 1) and benign pathology was found (Group 2). Students t-test was used for statistical analysis. Results: According to biopsy findings, prostate cancer was detected in 74 patients while no cancer was detected in 418 patients. The average age of Group 1 was calculated to be 65±8.6, while the average age of Group 2 was 64±8.1. The average prostate volume in Group 1 was 45±23 cc, while in Group 2 it was found to be 58±26 cc. Conclusion: In line with the literature with the literature, the average prostate volume of the cancerous group was significantly lower. This supports the notion that cancer detection is more probable in small prostates than in high-volume prostates.
