Yüzeyel mesane tümörlü hastaların tanı ve takibinde NMP22 testinin değerliliği

Özet
Amaç: Bu çalışmada nükleer matriks protein 22 (NMP22) testinin teşhiste ve yüzeyel mesane tümörü tanılı hastaların takibindeki etkinliği araştırıldı. Gereç ve yöntemler: Hematüri saptanmış veya mesane tümörü tanısı olup kontrol amaçlı başvurmuş olan hastalar prospektif olarak çalışmaya dahil edildi. Sistoskopi öncesi alınan idrar örneği ile sitoloji ve NMP22 testi çalışıldı. Mesane kanseri tanısı alan hastalar evre, derece, sayı, boyut ve risk açısından kategorize edi- lip NMP22 testi ile sitolojinin her biri için ROC eğrileri elde edildi ve iki testin tek başına veya kombine tanısal performans değerleri tanı ve takip için ayrı ayrı araştırıldı. Bulgular: Hematüri nedeni ile araştırılan grupta toplam 87 hasta olup NMP22 testinin duyarlılık oranı %70, özgüllük oranı %80, pozitif kestirim değeri %68, negatif kestirim değeri %81 ve pozitif test sonucu- nun olasılık (LR+) oranı 3,42 olarak bulundu. Sitolojinin duyarlılık oranı %27, özgüllük oranı %96, pozitif kestirim değeri %82, negatif kestirim değeri %68 ve pozitif test sonucunun olasılık (LR+) oranı 7,36 olarak bulundu. Mesane tümörü tanısı olup takip edilen grupta toplam 49 hasta olup NMP22 testinin duyarlılık oranı %33, özgüllük oranı %76, pozitif kestirim değeri %31, negatif kestirim değeri %78 ve pozitif test sonucunun olasılık (LR+) oranı 1,37 olarak saptandı. Sitolojinin duyarlılık oranı %25, özgüllük oranı %97, pozitif kestirim değeri %75, negatif kestirim değeri %80 ve pozitif test sonucunun olasılık (LR+) oranı 9,25 olarak saptandı. Sonuç: NMP22 testi, mesane kanseri tanısında sistoskopiye yardımcı olarak kullanılabilir. Mesane kanseri tanılı hastaların takibinde tek başına ya da idrar sitolojisi ile kombinasyonunun tanı koydurucu gücü sınırlıdır.
Abstract
Objective: In this study, we investigated the efficacy of nuclear matrix protein 22 (NMP22) in the diagnosis and surveillance of bladder cancer. Material and methods: Patients with a complaint of hematuria or who required cystoscopic control of prov- en bladder cancer were prospectively enrolled in this study. A routine cytologic examination and NMP22 test were performed on the voided urine sample that was provided before the cystoscopy. The patients who had been diagnosed with bladder cancer were categorized according to stage, grade, number, measure and risk, and the diagnostic performance of the NMP22 test and the cytology test, alone or in combination, were evaluated separately using ROC curves in the diagnosis and surveillance groups. Results: There were 87 patients (of the 136 total patients) who were investigated because of hematuria. The sensitivity, specificity, positive predictive value, negative predictive value and positive likelihood ratio (LR+) of the NMP22 test were 70%, 80%, 68%, 81% and 3.42, respectively. The sensitivity, specificity, positive pre- dictive value, negative predictive value and positive likelihood ratio (LR+) of the cytology examination were 27%, 96%, 82%, 68% and 7.36, respectively. There were 49 patients in the group that had been diagnosed with bladder cancer. The sensitivity, specificity, positive predictive value, negative predictive value and positive likelihood ratio (LR+) of the NMP22 test in these patients were 33%, 76%, 31%, 78% and 1.37, respectively. The sensitivity, specificity, positive predictive value, negative predictive value and positive likelihood ratio (LR+) of the cytology examination were 25%, 97%, 75%, 80% and 9.25, respectively.Conclusion: The NMP22 test can be used as an adjunctive tool for the detection of bladder cancer, but its diagnostic performance is limited in surveillance when used alone or in combination with a cytology examination.
