Septik ve nonseptik hastalarda APACHE II, APACHE III, SOFA skorlama sistemleri, trombosit düzeyleri ve mortalite

Özet
AMAÇ Bu prospektif çalışmada sepsisi olan ve olmayan yoğun bakımda tedavi görmekte olan yüz yirmi hastada Akut Fizyolojik ve Kronik Sağlık Durum Değerlendirmesi (Acute Physiology and Chronic Health Evakuation, APACHE II, APACHE III ve Sepsise İlişkin Organ Yetersizliği Değerlendirmesi (Sepsis-related Organ Failure Assessment, SOFA) skorlama sistemleri ile trombosit düzeylerinin prognostik değerinin araştırması amaçlanmıştır. GEREÇ VE YÖNTEM Bu çalışmaya toplam yüz yirmi yoğun bakım hastası dahil edilmiştir. Hastalar önce sepsis olan ve olmayan şeklinde, daha sonra sağkalanlar ve ölenler olmak üzere toplam dört gruba ayrılmıştır. Hastaların hastaneye yatış ve hastaneden çıkış gününde SOFA, APACHE II ve III skorları ile trombosit düzeyleri kaydedilmiştir. BULGULAR Hastalar septik sağkalanlar (n=42), sepsisten ölenler (n=14), nonseptik sağkalanlar (n=48) ve sepsis dışı nedenlerle ölenler (n=16) olmak üzere dört gruba ayrılmıştır. Sepsisten ölen hastalarda en yüksek SOFA, APACHE II ve III skorları ile en düşük trombosit düzeyleri elde edilmiştir. Ayrıca septik hastalarda mortaliteyi tahmin etmede SOFA, APACHE II ve III skorları ve trombosit düzeylerinin anlamlı olduğu bulunmuştur. SONUÇ Septik olan ve olmayan yüz yirmi yoğun bakım hastasında yürüttüğümüz bu çalışmada, septik olgularda mortaliteyi öngörmede hastaneye yatış ve hastaneden çıkış gününde saptanan SOFA, APACHE II ve III skorları ve trombosit değerinin kullanılabileceği kanısına varılmıştır.
Abstract
BACKGROUND: The aim of the current prospective study was to examine the prognostic value of APACHE II, APACHE III (Acute Physiology and Chronic Health Evakuation II and III) and SOFA (Sepsis-related Organ Failure Assessment) scores and platelet counts in septic and nonseptic patients hospitalized and treated in ICU units. METHODS: One hundred and twenty ICU patients were included in this study. Patients were grouped as septic survivors, septic nonsurvivors, nonseptic survivors and nonseptic nonsurvivors. The SOFA, APACHE II and III scores, and platelet counts were recorded at the admission in the ICU units and discharge.from the hospital. RESULTS: Patients were grouped as septic survivors (n=42), septic nonsurvivors (n=14), nonseptic survivors (n=48) and nonseptic nonsurvivors (n=16). The highest SOFA, APACHE II and III scores and the lowest platelet counts were observed in septic nonsurvivors when compared with the other patient groups.. All score values and platelet counts were found to be significant in prediction of mortality in septic patients. CONCLUSIONS: According to our results obtained in septic and non-septic patients, we concluded that SOFA, APACHE II and III scores and platelet counts might be used in the prediction of mortality in septic patients.
