Preoperatif kemoradyoterapi uygulanan rektum kanserli olgularda patolojik tam yanıtın önemi

Özet
Rektum kanseri tanısıyla preoperatif adjuvant kemoradyoterapi uygulanan olgularda patolojik tam yanıt (pTY) elde edilmesinin prognostik değerinin araştırılması amaçlanmıştır. Preoperatif kemoradyoterapi sonrası radikal rezeksiyon ile tedavi edilen evre II-III rektum kanserli 65 olgu değerlendirilmiştir. Rekürrens ve sağkalım, tümör özellikleri ve pTY ile kıyaslanmıştır. Tümör evresinin gerilemesi %77 olguda, pTY %20 olguda saptanmıştır. Klinik ve patolojik değişkenlerin hiçbirisi pTY ile ilişkili bulunmamıştır. 5 yıllık lokal rekürrens oranı %14 ve uzak metastaz oranı %26’dır. pTY alınan sadece bir olguda hastalık nüksü gözlenmiştir (uzak metastaz). Patolojik evresi I, II ve III olan olgular arasında hastalıksız sağkalım açısından fark saptanırken genel sağkalım açısından fark saptanmamıştır. Preoperatif kemoradyoterapi ile pTY sağlanması lokal kontrol ve hasta sağkalımında düzelme ile ilişkili bulunmuştur.
Abstract
The prognostic value of pathologic complete response (pCR) to preoperative chemoradiotherapy in rectal cancer patients was aimed to be evaluated. Sixty-five patients with stage II-III rectal cancer treated by preoperative chemoradiation followed by radical resection were evaluated. Recurrence and survival were compared with tumor characteristics and pCR. Tumor downstaging occured in 77%, including 20% who had a pCR. None of the clinical or pathologic variables were associated with pCR. The estimated 5-year rate of local recurrence was 14%, of distant metastasis, 26%. Only one of the patients with pCR has developed disease recurrence (distant metastasis). A significant difference was found in disease-free survival among patients with pathologic stages I, II or III tumors, but not in overall survival. A pCR to preoperative chemoradiation is found to be associated with improved local control and patient survival.
