Özet
Rektum kanseri her iki cinste de sık izlenen bir kanser türüdür ve primer tedavisi cerrahidir. Günümüzde postoperatif kemoradyoterapi sıklıkla tercih edilen adjuvan tedavi yaklaşımıdır fakat tedavi sonuçları halen ideal değildir. Yakın zamanlı olarak yapılan çalışmalar adjuvan kemoradyoterapinin postoperatif değil de preoperatif uygulanmasının total mezorektal eksizyon cerrahisi ile birlikte daha iyi lokal kontrol oranları sağladığını bildirmektedir. Radyoterapi, kemoterapi ve kemoradyoterapi şeklindeki adjuvan tedavilerin bu açıdan cerrahiye göre zamanlaması nasıl olmalıdır sorusu yeterli hasta sayılarının kullanıldığı randomize klinik çalışmalar ışığında tartışılmıştır. Sonuç olarak preoperatif eş zamanlı kemoterapi ve radyoterapinin tercih edilmesi önerilmektedir.
Abstract
Cancer of the rectum is a frequent malign disease of both sexes and its primary treatment is surgery. Postoperative chemotherapy is the generally preferred adjuvant treatment modality but its results are not still satisfactory. Recent studies have reported that total mesorectal excision technique with preoperative chemoradiotherapy is superior to postoperative adjuvant treatment in terms of local disease control. The question of timing of the adjuvant treatments like radiotherapy, chemotherapy and chemoradiotherapy will be discussed in light of the randomized clinical trials with sufficient patient number. In conclusion, current recommendation is preoperative concomitant chemotherapy plus radiotherapy.
