Covid-19 Pandemisi Öncesi ve Sonrası Uygulanan Jinekolojik Onkolojik Cerrahilerin Kısa Dönem Sonuçlarının Karşılaştırılması

Özet
Giriş:Çalışmanın amacı Covid-19 pandemisi sırasında opere edilen jinekolojik onkoloji olgularının kısa dönem sonuçlarının pandemi öncesiyle karşılaştırılması.Gereç ve yöntemler:Bu retrospektif vaka kontrol çalışması Başkent Üniversitesi Jinekolojik onkoloji kliniğinde yürütülmüştür. 11 Mart 2020 öncesi ve sonrası opere edilen olgular 2: 1 olacak şekilde , kanserin tipi,evre ve yaş açısından eşleştirilmiştir. Vakaların kliniko-patolojik özellikleri,intraoperatif ve postoperatif kısa dönem sonuçları karşılaştırılmıştır. Bulgular:Covid- 19 sonrası 41 hasta bulunurken,covid-19 öncesi grupta 82 hasta bulunmaktadır.Pandemi sırasında en çok opere edilen kanser endometrium kanseri idi (19/41) bunları sırasıyla over kanseri (18/41) serviks kanseri (3/41) ve sarkom (1/41) izledi . Gruplar arasında grad, cerrahilerin tipleri, lenf nodu sayıları,komorbidite ve preoperatif CA 125 seviyeleri arasında fark izlenmedi (p değerleri sırası ile 1-0,1-0.08-0.09-0.09-0.33). Iki grupta da intra ve post operatif komplikasyonlar benzerdi. Yatış süresi ,transfüzyon ve tedaviye başlama süresi açısından iki grupta fark izlenmedi(p değerleri sırası ile 0.6-0.4-0.7-0.7) Sonuç: Uygun şartlar altında stardart tedavisi cerrahi olan olgularda operasyonlar devam etmelidir.
Abstract
Introduction:We aimed to compare short term results of gynecologic oncology patients operated before and during Covid-19 pandemic. Material and Methods:This retrospective study was conducted in Baskent University School of Medicine.11 March 2020,has been taken the cut of time.Patients were devided into two groups according to this particular time.Case and control groups were matched as1:2. Variables like age,cancer and stage were matched and clinic-pathologic features,intra and post operative results were compared. Results:There were 41 patients in the post-covid- 19 group,and 82 patients in the pre-covid- group.The most common gynecologic cancer was endometrial cancer(19/41),the rest were ovarian cancer( 18/41),cervical cancer(3/41 )and sarcoma(1/41).There were no difference between groups regarding grade,surgery,lymph node numbers,comorbidity and CA 125 levels (p=1-0,1-0.08-0.09-0.09-0. 33respectively).Intra and post-operative complications were similar. There was no difference in the two groups in terms of hospital stay,transfusion, and timing of adjuvant therapy (p=0.6-0.4-0.7-0.7). Conclusion:Surgeries should continue under appropriate conditions, especially in cases which surgery is the first choice of treatment
