Hasta kontrollü analjeziye ondansetron veya metoklopramid eklenmesi

Özet
Bu çalışmada genel anestezi altında jinekolojik ameliyat veya laparoskopi yapılmış 60 kadın hasta rast gel e 20şerlik üç gruba ayrılarak kendilerine ya sadece meperidin [111. (plasebo)] veya meperidin + antiemetik kombinasyonu (I. ve 11. gruplar) ile hasta kontrollü analjezi uygulanmış, antiemetik olarak I. grupta ondansetron, II. grupta metoklopramid kul¬lanılmıştır. Meperidinin bolus dozları 20 mg, on¬dansetron ve metoklopramidinkiler ise sırasıyla 0,5 ve 2,7 mg ve "lockout" 5 dakika olmak üzere ayar¬lanmıştır. Ameliyattan sonraki ilk 24 saat zarfında kullanılmış olan ondansetron dozlarının 2,9 ± 0,9 mg ve metoklopramid dozlarının 14,5 ± 4,3 mg olduğu izlenmiştir (ortalama ve standart sapma). Antiemetik gruplarında bulantıdan şikayet eden hasta sayısının önemli derecede daha az olduğu (ondansetron grubunda P<0,01; metoklopramid grubunda p<0.05 plasebo ile karıştırma) sap¬tanmıştır. Hastalarda yan etkiler izlenmemiştir. Has¬taların ifadelerine göre, hasta kontrollü analjezi ile antiemetiklerin kombinasyonu halinde daha mükemmel bir analjezi sağlanabildiği kanısına varılmıştır.
Abstract
In this study, 60 women who had undergone gynecologic operations or laparoscopies under ge¬neral anaesthesia were allocated randomly into three groups (which had 20 patients each) to receive patient-controlled analgesia with either meperidine combined with antiemetics (in group one with on¬dansetron and group two with metoclopramide) or meperidine alone (group 3-placebo). Bolus doses of meperidine 20 mg, and ondansetron 0,5 mg or me¬toclopramide 2,7 mg were used with a lockout time of 5 min. During the first 24 h after surgery the mean (range) dose of ondansetron in the on¬dansetron group was 2,9 ± 0,9 mg and me¬toclopramide in the metoclopramide group 14,5 ± 4,3 mg. Significantly fewer patients in ondansetron and metoclopramide groups had nausea (P<0,01 and 0,05 respectively). Side effects were not ob¬served in any patient. Significantly more patient had the opinion that patient controlled analgesia had provided excellent analgesia when antiemetics were used.
