Anestezi idamesinde midazolamın iki farklı yöntemle kullanımı ve flumazenilin uyanmaya etkileri

Özet

Çalışmaya batın cerrahisi ameliyatı geçirecek ASA l-ll 40 hasta alındı. Hastalar rasgele iki gruba ayrıldı. Anestezi idamesinde midazolam 0.1mg/kg bolus (grup l) (n=20) ve 0.1mg/kg/saat sürekli infüzyon şeklinde (grup II) (n=20) iki farklı yöntemle uygulanarak hemodinamik parametrelere etkileri araştırıldı. Postoperatif kullanılan flumazenilin her iki gruptaki uyanma ve hemodinamiye etkileri de karşılaştırıldı. Tüm olgulara İM 0.07mg/kg midazolam+O.5 mg atropin ile premedikasyon uygulandı. İndüksiyonda iv 0.3mg/kg midazolam, 0.5mg/kg atracurium, idamede her iki grupta %50N2O+%502 ve iv 1mg/kg meperidin kullanıldı.İndüksyondan 1 dakika sonra, entübasyon sonrası, cerrahi insizyon ve peroperatif 10 dak. ara ile SAB, DAB, KAH kaydedildi. SpO2 sürekli izlendi. Ekstübasyondan 1 dakika sonra Uyanıklık/Sedasyon Değerlendirme Skalası ile uyanma skorları saptandı. İV 0.2mg flumazenil verildi. Daha sonra 1'er dakika ara ile skor 1 oluncaya dek 0.1 mg dozda flumazenil tekrarlandı. İstatistiksel değerlendirmede Student's t testi kullanıldı (p<0.05 anlamlı kabul edildi). Hemodinamik bulgular her iki grupta stabil seyretti, Grup II'de kullanılan total midazolam dozu anlamlı düşük bulundu ve hastalar daha hızlı uyandılar. Genel anestezi idamesinde fraksiyone bolus veya sürekli infüzyon midazolam yeterli ve stabil anestezi sağlamaktadır. Uyanmanın daha hızlı gerçekleşmesi ve midazolam total dozunun anlamlı düşük olması nedeniyle anestezi idamesi için infüzyon midazolam yönteminin tercih edilmesini önermekteyiz.

Abstract

Midazolam for maintenance of anaesthesia: two different methods and the effects of flumazenil on emergence 40 ASA l-ll patients undergoing abdominal surgery were included in the study. They were randomly allocated into two groups. Midazolam was used for maintenance. Group I (n=20) recieved iv 0.1mg/kg bolus increments as needed while group II (n=20) recieved 0.1mg/kg/hr continious infusion. The effects on hemodynamics was studied. The effects of postoperative flumazenil on emergence and hemodynamics were also evaluated. All patients were premedicated with İM 0.07mg/kg midazolam+0.5mg/kg atropine. 0.3mg/kg midazolam, 0.5mg/kg atracurium was used for induction. Maintenance was supplemented with 50%O2+50%N2O and iv 1mg/kg meperidine in both groups. The SAB,DAP,HR were recorded at the first minute after induction, intubation, incision and every 10 minutes perioperatively. SpO2 was monitored continiously. One minute after extubation the alertness was assesed with the Alertnes-Sedation Scale and iv 0.2mg flumazenil was given. Flumazenil was repeated every minute until the score became 1. Student's t test was used for analysis (p<0.05 was considered as significant). The hemodynamic parameters were stable in both groups. The total midazolam dose was sigificantly lower and emergence was more rapid in group II. Either incremental bolus or infusion midazolam provides stable and satisfactory anaesthesia maintenance. Because of lower midazolam dosage and more rapid emergence, the infusion method is more preferable.
