Renal arter hastalıklarının tanısında renkli Doppler ultrasonografinin değeri

Özet
Hipertansiyonun ■ rcnovasküler nedeninin sap¬tanmasında renal arterlerin değerlendirilmesi büyük önem taşımaktadır. Bu çalışmada remil arterde % 50 veya daha fazla çap daralması yapan anlamlı ste-nozları saptamada renkli doppler ultrasonografinin (RDU) etkinliği araştırıldı. Çalışma Mart 1993 -Aralık 1994 tarihleri arasında Ankara Üniversitesi Tıp Fakültesi Üroloji ve Radyoloji. Anabiliın Dal¬larında yapıldı. 20 hipertansif'hastada renkli dopp¬ler ultrasonografi bulguları renal anjiografi bul¬guları ile karşılaştırıldı. Anjiografi ile bu 20 hastada 39 ana ve 4 aksesuar renal arter gösterildi. An¬jiografi 5 ana renal arterde ve 1 aksesuar renal ar¬terde stenozu gösterdi. 4 ana renal arterde oklüzyon saptandı. Tüm hastalarda RDU ile renal arter mak¬simum sistolik hız, renal arter/aorta maksimum sis-tolik hız oranları (RAR) hesap edildi. Bir hasta için RDU ile ortalama inceleme süresi 42+4 (38 - 53) dakikaydı. RDU ile ana renal arterlerin % 87.1 'i ve sadece 1 (%25) aksesuar arter gösterildi. Mak¬simum sistolik hızın 100 cmlsn'nin üstünde olması ve RAR değerinin 3.5 veya üstünde olması stenoz için kriter olarak seçildi. Renal arter stenozıınun tanısında birinci kriter daha faydalıydı. Bu kritere göre, RDU ile 6 renal arter stenozıınun tanısında % 67 sensitivite ve % 93 spesifisite bulundu. Renkli . doppler ultrasonografinin, kullanılan ekipman ve kriterlerle anjiografiye bir alternatif olmadığı ve renal arter stenozıınun tanısında renkli doppler ultrasonografinin etkinliğini arttırmak için teknolojisinin iyileştirilmeye ve tanı kriterlerinin standardize edilmesine, ihtiyaç olduğu kanısına varıldı.
Abstract
The assesment of the renal arteries is par¬ticularly important in the detection of a re-novascular cause of hypertension. It was in¬vestigated the effectiveness of color doppler ultrasonography (CDU) in the detection 50 % or greater diameter reducing the stenosis in renal ar¬tery in this study. The study was done in the De¬partment of Urology and Radiology, University of Ankara, Medical School between March 1993 and December 1994. The findings of color doppler ult¬rasonography in 20 hypertensive patients were com¬pared with the results of renal angiography. It was demonstrated 39 main and 4 accessories renal ar¬teries in these 20 patients by angiography. An¬giography demonstrated the stenosis in 5 main renal arteries and in one accessory renal artery. It was determined the occlusion in 4 main renal arteries. The maximum systolic velocity and the ratio of ma¬ximum systolic velocity in renal artery to that in ab¬dominal aorta (RAR) were calculated with CDU in all patients. The average procedure time per pa¬tients with CDU was 42+4 (38 + 53) minutes. It was demonstrated 87.1% of the main arteries and only 1 (25%) of the accessory vessels by CDU. A maximum systolic velocity value of greater than 100 cm/sec and a RAR value of 3.50 or greater were chosen as the criteria for stenosis. The first criteria in the di¬agnosis of renal artey stenosis was more useful. Ac¬cording to this criteria, we found 67 % sensitivity and 93% specificity in diagnosing 6 renal artey ste¬nosis with CDU. We concluded, that CDU is not an alternative to angiography with the technology and criteria used, and the technology of CDU should be improved and the diagnostic criterias should be standardized to increase the effectiveness of CDU in diagnosis of renal artery stenosis.
