Üreter alt kısım taşlarının tedavisinde ESWL ile üreteroskopinin etkinliğinin karşılaştırılması

Özet
Alt üreter taslarinda ilk tedavi seçeneginin ESWL (Ekstrakorporel sok dalgalari ile litotripsi) mi, yoksa Üreteroskopi mi olmasi halen tartismalidir. Alt üreter tasi nedeniyle 321 hastaya ESWL uygulandi. 131 olguda lokalizasyon ultrasongrafi (Group I), 186 olguda da floroskopi (Group II) ile yapildi. 121 hasta ise üreteroskopik girisimle tedavi edildi. (Group III). ESWL grubunda total basari orani %85 iken bu oran üreteroskopik girisim grubunda %94.2 olarak bulundu. (P=0.015) Grup I'de % 91.1, grup II'de ise %80.6, basari oranlari vardi. (P=0.015) Grup I'de tekrarlanan tedavi orani (%11.8), grup II'ye göre belirgin olarak düsük (%26.3) bulundu. (P=0.001) ESWL gruplarindaki basari üreteroskopik girisimdeki basari ile karsilastirildiginda üretroskopik girisim Grup I ile anlamli farklilik göstermez iken (P=0.48), Grup II'ye göre daha basarili bulundu. (P=0.001) Üreteroskopik girisim grubunda (%13.2), ESWL grubuna (%3.7) göre daha yüksek oranda komplikasyon görüldü (P=0.004). Ultrasonografi ile görüntülenebilen alt üreter taslarinda ESWL'nin tatminkar basari oranlari ve komplikasyon oranini düsüklügü nedeni ile ilk tercih edilmesi gereken tedavi yöntemi oldugu sonucuna varildi.
Abstract
Selection of primary mode of therapy ofor lower ureteral stones is still controversial,should it be ESWL (Extracorporeal shock wave lithotripsy) or ureteroscopic approach? ESWL therapies were applied to 321 patinets. Ultrasonographic targetings were used when stones could be localised by ultrasound in 131 patients (Group I), fluoroscopic targetings were used in 186 patients (Group II) during ESWL treatment. Uretroscopic approach was selected in 12 1patients as a primary treatment of lower ureteral stones. (Group III) Total success rates were 85% for ESWL treatment and 94.4% for ureteroscopic approach. (P=0.015) Success rates were 91.1% and 80.6% in Group 1 and 2 subsequently. (P=0.015) Retreatment rate was clearly lower in group I (11.8%) than the group II (26.3%). (P=0.01) Success rate of ureteroscopic approach was compared to ESWL groups, while there was no difference with group I, (P=0.048) success rate of grup III was higher than group II. (P=0.002) Complication rates showed significant difference between ESWL and ureteroscopy groups. (13.2% versus 3.7%) (P=0.004) We concluded that ESWL should be selected as a primary therapy of lower ureteral stones that can be seen by ultrasound due to it's the higher success and lower complication rates than the ureteroscopic approach.
