Atopik bronşiyal astmalı olgularda prick testi ve spesifik IgE sonuçlarının değerlendirilmesi

Özet
Klinigimizde yapilan bu çalismada astmali hastalarda cilt testi, serum total ve spesifik IgE düzeyleri karsilastirilmis ve tanisal degerleri incelenmistir. Ayrica prick testinde saptanan farkli her allerjen için serum total ve spesifik IgE degerlerinin sensitivite ve spesifiteleri hesaplanmistir. Çalisma ve kontrol grubuna prick testi yapilmis ve her iki grubun total IgE düzeyleri saptanmistir. Prick testte pozitiflik gözlenen allerjenler için spesifik IgE düzeyleri ölçülmüstür. Allerjik astmalilarin serum total IgE düzeyleri (301.7±412.1 IU/ml) kontrol grubu (77.1±55.3 IU/ml) ile karsilastirildiginda anlamli olarak yüksek bulunmustur (p 0.05). Serum total IgE sensivitesi %57 olarak hesaplanmistir. Serum spesifik IgE düzeyini sensivitesi %75-100 arasinda ve spesifitesi ise %100 (ev tozu akarlari hariç) olarak tespit edilmistir. Ayrica serum total IgE yüksekligi ile serum spesifik IgE müspetligi arasinda zayif korelasyon gösterilmistir (r=0.25). Sonuç olarak her ne kadar çalisma grubunda total IgE düzeyleri kontrol grubundan anlamli olarak yüksek bulunsa da, bu yüksekligin atopinin tanisinda sensitif olmadigi gösterilmistir. Cilt prick test sonuçlari ile paralellik gösteren serum spesifik IgE düzeylerinin, atopik astmalilarda rutin olarak arastirilmasinin gerekmedigi kanisina varilmistir.
Abstract
In this study; skin prick test, serum total and spescific IgE levles were compared in patients with asthma and the diagnostic values of these tests wer einvestigated. In addition sensivity and specifity of total and specific IgE values were calculated for different allergen groups. Skin prick tests were applied to asthmatic and control cases and serum total IgE levels were determined in both groups. In asthmatic and control groups, serum specific IgE levels were determined for allergens which were observed positive in prick test. The serum total IgE levels of asthma group (301.7±412.1 IU/ml) were significantly higher when compared to the control group (77.1±55.3 IU/ml). Sex and family history of atopy were not factors influencing the total IgE levels in asthmatic patients (p>0.05). The sensitivity of serum total IgE was 57%. The sensitivity and specifity of serum specific IgE was 75-100% and 100% (except for mites), respectively. The correlation between high serum total IgE and serum specific IgE positivity was weak (r=0.25). We concluded that high serum total IgE levles are not sensitive in diagnosis of atopy in asthmatic patients. Serum specific IgE levels should not be investigated in atopic patients as routine, because of it has high specifity with allergens positive in skin prick tests.
