Özet

Bu çalışmada diyabetik ayak yarasına tedavi amaçlı konulan greft zemininde gelişen Malign Fibroz Histiyositoma (MFHj olgusu sunuldu. Diyabetik hastaların ayaklarında görülen yaralar genellikle diyabete bağlı iyileşmeyen kronik ülserler olarak yorumlanır. 12 yıldır Tip 2 diyabeti olan 48 yaşında bayan hasta ayak I. parmak medial yüzünde yaklaşık 2x2 cm'lik kronik yara ile kliniğimize başvurdu. Mevcut doku granülasyon dokusu olarak değerlendirilip eksize edildi. Yaklaşık bir aylık takip sonunda yeterli epitelizasyon olmayan yaraya lokal anestezi altmda ince kalınlıkta deri grefti konuldu. Zaman içerisinde greftte rezorbsiyon görüldü ve granulasyon dokusuna benzer bir doku gelişti. Doku eksize edilerek histopatolojik incelemeye alındı ve malign fibroz histiyositoma tanısı konuldu. Olguya ray amputasyon uygulanarak tedavi gerçekleştirildi. Postoperatif dönemde komplikasyon olmadı, 2 yıllık takipte nüks görülmedi. Literatürde greft zemininde gelişen MFH olgusuna ilk kez rastlanılmaktadır.

Abstract

In this study we presented a case whose Malign Fibrous Histiocytoma (MFH) was occured in the base of the graft which was implanted in the foot of the diabetic patient. The wounds of the foot in diabetic patients are usually thought to be cronic ulcers related to diabetes. A 48 years old female patient with diabetes mellitus since 12 years applied to our clinic with 2x2 cm cronic wound on medial site of her first finger of the foot. The lesion was thought as granulation tissue and if was excised. After one month postoperatively split thickness skin graft was used for reconstruction because of in sufficient epithelization. Resorbtion was seen on the skin graft during the healing period and a new tissue appeared like granulation tissue. By the time graft was resorbed and a new tissue like granulation developed. This tissue was excised and the result of histopatho-logical examination was malignant fibrous histiocytoma. The patient was cured with ray amputation. There were no complication and no recurrence at the 2nd year postoperatively. This is the first case report in whom MFH occured in the base of the base of the graft of diabetic foot ulcer.
