Özet

Deri tüberkülozu (tbc), derinin kronik seyirli granülomatöz enfeksiyonudur. Etkenin deriye giriş yoluna, konakçının immün yanıtına, basilin sayı ve virulansına göre değişik klinik formlarda ortaya çıkabilir. Primer inokulasyon tüberkülozu (tbc şankırı) daha önce basil ile teması olmamış kişilerin derisine basillerin ekzojen yolla alınmasıyla ortaya çıkar. 62 yaş kadın olgu iki aydır olan sağ koltuk altında lenfadenopati ve sağ kolda sporotrikoid olarak yerleşmiş mor menekşe renkli nodüller şikayeti ile cildiye polikliniğine başvurdu. Histopatolojik incelemede; fokal nekroz alanları içeren granulomatöz inflamasyon ve langhans tipi dev hücreler görüldü. Diğer ayırıcı tanılar dışlandıktan sonra olguya klinikopatolojik olarak primer inokülasyon tüberkülozu tanısı konuldu. Antitüberküloz tedavi sonunda lezyonların postinflamatuar hiperpigmentasyon, hipopigmentasyon ve atrofik skatrisle tamamen gerilediği görüldü. Literatürde daha önce sporotrikoid paternde bildirilmiş bir tane primer tüberküloz olgusuna rastlanmıştır, olgumuz literatürdeki ikinci bildiridir. Burada, sporotrikoid paternde görülen primer inokülasyon tüberkülozlu bir olgu sporotrikoid paterndeki lezyonların ayırıcı tanısında akılda tutulması amacıyla sunulmuştur.

Abstract

Skin tuberculosis (tbc) is a chronic granulomatous infection of the skin. This can occur in different clinical forms depending on the route of entry, the immune response of the host, the number of the bacilli and the virulence. Primer inoculation tuberculosis (tuberculosis chancre) occurs in people with that have not previously been contacted with the bacilli. A 62-year-old woman presented to the dermatologic polyclinic with a complaint of lymphadenopathy in the right axilla for two months and purple violet nodules located as a sporotrichoid pattern in the right arm. The histopathological examination; granulomatous inflammation including focal necrosis areas and giant cells of langhans type were observed. After excluding the other differential diagnoses, the patient was diagnosed with primary inoculation tuberculosis with clinicopathologically. At the end of the antituberculosis treatment, the lesions completely disappeared with postinflammatory hyperpigmentation, hypopigmentation and atrophic scarring. In the literature, a primary tuberculosis case was seen in the sporotrichoid pattern, and in this way our case is the second report in the literature. Herein, a case with primary inoculation tuberculosis seen in sporotrichoid pattern is presented in order to keep in mind the differential diagnosis of lesions in sporotrichoid pattern.
