Serviks karsinomunun radyoterapiye cevabının ultrasonografi ile değerlendirilmesi: Manyetik rezonans bulguları ile rezistiv indeks korelasyonu

Özet
Amaç: Bu prospektif çalışmada serviks karsinomunun radyoterapiye cevabının değerlendirilmesinde transvajinal renkli doppler ultrasonografi ile ölçülen rezistiv indeksin rolünü incelemek ve manyetik rezonans bulguları ile karşılaştırmak amaçlanmıştır. Gereç ve yöntem: İleri evre (>IIA) serviks kanseri histopatolojik tanı alan 13 hastaya radyoterapi öncesi ve 6 ay sonrası MRG ve TVRDUS yapıldı. Tümör santral ve perifer kesiminden ölçülerek ortalaması alınan rezistiv indeks tedavi öncesi ve sonrası değerleri karşılaştırıldı. Bu değerler MRG bulguları ve kontrol grubunun rezistiv indeks değerleri ile karşılaştırıldı. Bulgular: B-mod transvajinal ultrasonografi (TVUSJ ile tedavi öncesi tüm hastalarda tümöral kitle görüntülenebildi.Tedavi öncesi ve sonrası rezistiv indeks değerleri sırasıyla 0.20-0.82(ortalama:0.52), 0.70-0.99 (ortalama:0.81) olarak ölçüldü. 13 hastadan 1 l'i tedaviye tam cevap verdi ve MR tetkikinde hiçbir kitle saptanmadı. TVUS ile yapılan incelemede servikste kitle gözlenmedi. 2 hastada ise MR tetkiki ve ultrasonografi ile rezidüel kitle izlendi. Tedaviye tam cevabı olanlarda radyoterapiye bağlı rezistiv indeks değerindeki artış istatistiksel olarak anlamlı bulundu (p=0.001). Rezidüsü olan 2 hastada ise rezistiv indeksler artmadı. Kontrol grubunun rezistiv indeks ortalaması 0.65 olarak ölçüldü ve hastaların tedavi öncesi rezistiv indeks değerinden istatistiksel olarak anlamlı olarak yüksekti. Sonuç: TVRDUS ile elde edilen spektral değerler serviks karsinomunun tedaviye cevabının değerlendirilmesinde MRG'ye iyi bir alternatif olabilir çünkü MRG bulguları ile rezistif indeks değerleri anlamlı korelasyon göstermektedir.
Abstract
Aim: To assess prospectively the role of resistive indicies (Rl) in determining the response to radiotherapy by transvaginal color doppler ultrasound (TVCDUS) and to correlate the results with MRI findings. Material and Method: 13 patients with advanced stage (>stage IIA) cervical cancer evaluated by MRI of the pelvis and TVUS before and 6 months after radiotherapy treatment. The mean resistive indicies before and after daiotherapy were measured from the periphery and the center of the tumor and were compared. These values were further correlated with both MR findings and Rl values of the control group. Results: B mode TVUS identified the mass of all patients before therapy. Rl measured 0.20-0.82 (mean=0.52) and 0.70-0,99 (mean=0.81) before and after the radiotherapy respectively in 13 patients. 11 showed full response clinically and accordingly. MR showed no residual tumor. MR examination and US showed residual tumor in two patients. The increase in Rl before and after the radiotherapy was statistically significant (p=0.001) in the full response group. Rl did not increase in two patients with residua. The mean Rl measured from the control group was 0.65 and was significantly higher than the Rl of the patients measured before therapy. Conclusion: Spectral features of TVCDUS can be a good alternative in the follow up response of cervix carcinoma to therapy, since a significant correlation between MRI findings and Rl measurements are found.
