Özet

Amaç: Streptokoksik farenjit tanısında Breese skorlama sisteminin etkinliğini araştırmak ve bu skor­lama sisteminin tanısal değerini tespit etmektir. Gereç ve Yöntem: Bu çalışma sırasında akut ton-sillofarenjiti olan hastalar, A grubu beta hemolitik Streptokok (AGBHS) üreme durumuna göre 2 gruba ayrıldı (Grup I: üreme olanlar; Grup II: üreme olmayanlar). Her iki grupta da beyaz küre, C-reaktif protein (CRP) ve eritrosit sedimentasyon hızı (ESH) değerleri kaydedildi. Bulgular: Breese skorları Grup I' de. Grup ll'den daha yüksekti, ancak aradaki fark istatistiksel olarak anlamlı değildi. Breese skorlama sistemi için duyarlılık %31, özgüllük %74, pozitif tanımlama oranı %29, negatif tanımlama oranı %24, doğruluk oranı ise %63 olarak tespit edildi. C-reaktif proteini pozitif olan hasta­larda Breese skorlama sistemi için duyarlılık %56, özgüllük %41, pozitif tanımlama oranı %43, negatif tanımlama oranı %45, doğruluk oranı ise %48 olarak tespit edilirken; eritrosit sedimantasyon hızı yüksek olan hastalarda ise Breese skorlama sistemi için duyarlılık %50, özgüllük %48, pozitif tanımlama oranı %43, negatif tanımlama oranı %44, doğruluk oranı %49 olarak tespit edildi. Sonuç: Breese skorlama sisteminin yanı sıra CRP ve ESH gibi akut faz reaktanlarının kullanılması, AGBHS'ların neden olduğu farenjitin tanısının doğruluğunu artırmaktadır.

Abstract

Aim: Determination the efficiency and diagnostic reliability of Breese scoring systems in diagnosis of Streptococcal pharyngitis. Material and Method: This study consisted two groups. In Group I; patients were positive for isolation of group-A beta hemolytic Streptococcus from throat swabs, group II; patients were negative for isolation of group-A beta hemolytic Streptococcus for throat swabs. White blood cell counts, C-reactive protein and erythrocyte sedimentation rate levels were studied in two groups. Results: Breese score were higher in Group I than Group II, but this differ­ence was not significant. Sensitivity, specificity, positive predictivity, negative predictivity and diag­nostic value of Breese scoring system were 31 %, 74%, 29%, 24% and 63% respectively. Whereas sensitivity, specificity, positive predictivity, negative predictivity and diagnostic value of Bresse scor­ing at patient with high C-reactive protein was 56%, 41 %, 43%, 45% and 48% respectively. Finally,sensitivity, specifity, positive predictivity, negative predictivity and diagnostic value of Bresse scoring at patient with high erythrocyte sedimentation rate 50%, 48%, 43%, 44% and 49%, respectively. Results: Breese scoring system with the aspect of the acute phase reactants like C-reactive protein and erythrocyte sedimentation rate can increase the reliability of the diagnosis of group-A beta hemolytic Streptococcal pharyngitis.
