Özet
Bu çalışmada, Ocak 1996-Temmuz 2000 tarihleri arasında klinik olarak Tietze sendromu tanısı konularak izlenen 24 hasta retrospektif olarak değerlendirildi. Olgularda ortalama yaş 21.2 olup, 23'ü erkek ve 1 tanesi kadındı. Hastaların 8 (%33.3)'i medikal tedavilerden fayda gördü, diğer 16 (%66.6) hasta cerrahi olarak başarı ile tedavi edildi. Tanı hastalardan alınan anamnez, fizik muayene ve klinik değerlendirmeler sonucunda konuldu. Spesifik laboratuvar ve radyolojik bulgu tespit edilemedi. Olguların postoperatif patolojik incelemeleri kronik inflamatuvar periostal reaksiyon olarak değerlendirildi. Hastalar ortalama 5 ay (2-7 ay) takip edildi. 12 (%75) olguda ameliyat sonrası şikayetler tamamen ortadan kalktı, 3 (%19) hasta operasyon öncesine göre daha iyi olduğunu ifade etti ve 1 (%6) olguda operasyon sonrası şikayetlerde azalma olmadı. Sonuç olarak, Tietze sendromu tespit edilmiş, medikal tedaviye cevap vermeyen olgularda kostokondral rezeksiyon başarılı bir tedavi yöntemidir.
Abstract
In this study, between January 1996-july 2000, 24 patients with clinical suspicion of Tietze's syndrome were evaulated retrospectively. These 24 patients who were 23 men and 1 woman with a mean age of 21.2 years. Eight (33.3%) of the patients were treated medically and 16 (66.6%) of the patients were operated succesfully. The diagnosis is usually made on history of the disease, physical findings and clinical grounds. There were not specific roentgenographic and laboratory findings for diagnosis. Postoperative pathologic diagnosis of all cases were chronic inflamatory periostal reaction. The patients were followed up mean 5 months (2-7 months). Signs and symptoms disappeared completely after operation in 12 (75%), was relieved significantly in 3 (19%), and was not relieved at all in 1 (6%) patients. In conclusion, the patients with clinical evidence of Tietze's syndrome who couldn't treated medically, the choice of treatment is costochondral resection.
