Özet
Çalışmanın amacı; masif pulmoner tromboembolide (mPTE) trombolitik tedavinin etkinliğini değerlendirmek olup, bu amaçla primer değerlendirme kriterleri olarak majör kanama, rekürrens tromboemboli ve ölüm alındı. 01 Ocak 1996-20 Kasım 2001 tarihleri arasında kliniğimizde 193 hasta akut pulmoner tromboemboli (PTE) tanısı aldı. Bu hastaların 26 (%13.4)'sı mPTE tanısı alıp çalışma grubunu oluşturdu. Çalışma grubunu oluşturan hastaların 16'sı kadın 10'u erkek olup yaşları 22-85 yıl arasında değişiyordu (ortalama yaş: 60 ± 1 7 yıl). Tüm hastalara trombolitik tedavi uygulandı [20 hastaya doku plazminojen aktivatörü (TPA) ve 6 hastaya streptokinaz verildi], Trombolitik tedavi sonrasında antikoagülan tedaviye geçildi. Primer değerlendirme kriterleri, tedavinin 8. günü ile 90. gününde ele alındı. Tedavinin ilk 8 gününde; 1 hasta (%3.8) TPA uygulamasının 90. dakikasında gelişen majör kanama (intrakranial kanama) nedeni ile öldü. Bu dönem içinde rekürrens tromboemboli gözlenmedi (%0.0). Dokuzuncu gün ile 90. gün arasında; bir hasta (%3.8) bilinmeyen bir neden ile 14. günde ölürken, majör kanama ve rekürrens tromboemboli oluşmadı. Çalışmanın sonunda (90. günde) 1'i majör kanama (%3.8) nedeni ile olmak üzere 2 hasta ölürken (%7.6) rekürrens tromboemboli hiçbir hastada gözlenmedi (%0.0). Trombolitik tedavi mPTE tanısı alan hastalarımızda etkin ve güvenli bir tedavidir.
Abstract
To determine the effectiveness and safeness of thrombolytic therapy in patients with massive pulmonary thromboembolism (mPTE) with regard to primary evaluation criteria (major bleeding, recurrent thromboembolism and death). One hundred ninety-three patients were diagnosed as pulmonary thromboembolism (PTE) in our clinic between 01 January 1996-20 November 2001. Among them twenty-six patients (13.4%) were diagnosed as mPTE. The study group, mPTE cases, included 16 female and 10 male patients aged 22 to 85 years with a mean age of 60 ± 17 years. Thrombolytic drugs were given to all patients [tissue plasminogen activator (TPA); 20 patients, streptokinase; 6 patients]. Anticoagulant treatment was begun after administration of thrombolytic therapy. We monitored thrombolytic treatment at day eight and at day ninety with respect to the primary evaluation criteria. In the first eight days of treatment, one of twenty-six patients (3.8%) died at 90th minute of TPA administration due to major bleeding (intracranial bleeding). There was no recurrent thromboembolism within first eight days. Between day nine and ninety, one patient (3.8%) died on 14th day due to an unknown cause, but there was neither major bleeding nor recurrent thromboembolism in this period. Thus, at the end of the study (at day ninety) two patients died (7.6%), one of them was due to major bleeding (3.8%). There was no recurrent thromboembolism (0.0%) within ninety days. Thrombolytic therapy appears to be both effective and safe in our patients with a diagnosis of mPTE.
