Özet
Daha önce hiçbir tedavi görmemiş opere edilemeyen, evre 3B ve evre 4 küçük hücreli dışı akciğer kanseri (KHDAK) olan 4'ü kadın, 20'si erkek toplam 24 hasta paklitaksel ve sisplatin kombine kemoterapi (KT)'sinin aktivitesi ve toksisitesinin değerlendirilmesi için çalışmaya alındı. Çalışma kapsamına alınan 24 olgunun 23'ünde yanıt değerlendirilebildi. Median yaş 53.5 (29-63) yıl idi. Bu hastalara 21 gün arayla paklitaksel 200 mg/m2 ve sisplatin 80 mg/m2 kombine KT uygulandı. Yanıt alınan veya stabil seyreden hastalara maksimum 6 kür KT uygulandı. Hiçbir olguda tam yanıt alınamadı, 9 olguda (%39.1) kısmi yanıt alındı, 4 olgu (%17.5) stabil seyretti, 7 olguda (%30.4) progresyon ve 3 olguda (%13) erken progresyon saptandı. Median izlem süresi; 10 ay (2-16+ ay), median yanıt süresi 4.2 ± 4.2 (2-14+ ay) idi. Progresyona kadar geçen süre median 6 ± 4.7 ay (2-16) bulundu. Median sağkalım süresi 10 ± 4.5 (2-16) ay saptandı. Onaltı hasta primer hastalıklarının progresyonu nedeni ile öldü, 24 hastaya uygulanan toplam 100 kür KT siklusunda grade 3 nötropeni sadece 3 siklusta gözlendi, 1 siklusta ise febril nötropeni gelişti. Hematolojik olmayan yan etkiler; alopesi, bulantı, kusma, nefrotoksisite, periferik nöropati ve anemi idi. Bu çalışma sonucunda, paklitaksel sisplatin KT kombinasyonunun ileri evre KHDAK tedavisinde yan etkileri tolere edilebilecek düzeyde aktif bir rejim olabileceği düşünüldü.
Abstract
Twenty men, 4 women; total 24, stage 3B and stage 4 nonsmall cell lung cancer (NSCLC ) patients who had no previous therapy and who were inoperable at the time of presentation entered in a study for the assessment of activity and toxicity of paclitaxel and cisplatin combination chemotherapy. Among 24 patients, in 23 of them response can be evaluated in 23. In the study, median age of patients was 53.5 (29-63) years, combination chemotherapy of paclitaxel at a dose of 200 mg/m2 and cisplatin at a dose of 80 mg/m2 were administered with 21 days (3 weeks) intervals. Therapy was continued until six courses in responders. Of 24 patients, 9 had partial response (39.1%), 4 had stable disease (17.5%), 7 had disease progression (30.4%) and 3 had early progression (13%). Median follow-up period was 10 (2 to 16+) months and median response rate was 4.2 ± 4.2 months (range 2-14+ months). The time to progression ranged between 2 to 16 months with a median of 6 ± 4.7 months. Median survival rate of the patients was 10 ± 4.5 months (ranging between 2-16 months). During the study 16 patients died due to progression of their primary disease. Grade 3 neutropenia was detected in only 3 chemotherapy courses of totally 100 chemotherapy courses in 24 patients. Febrile neutropenia developed in 1 course. The other side effects observed were alopecia, nausea, vomiting, nephrotoxicity, neuropathy As a result of this study it was thought that paclitaxel and cisplatin combination therapy can be an active protocol with tolerable side effects in advanced NSCLC.
