Özet
Çalışmamızda, sarkoidozlu olgularımızın klinik, laboratuvar parametreleri ve solunum fonksiyon testlerinin değerlendirilmesi ve radyolojik evre ile bu parametreler arası ilişkinin incelenmesi amaçlanmıştır. Bu retrospektif çalışmada, 1996-2000 yılları arasında kliniğimizde izlenen 40 (kadın/erkek oranı: 28/12, yaş ortalama 40 ± 13 yıl) sarkoidoz olgusu incelendi. En sık rastlanan semptom öksürük (26 olgu, %65) idi. iki olguda aile anamnezi mevcuttu. Yirmiyedi olgunun 6 (%22)'sında üveit veya korioretinit, 10 olguda (%25) eritema nodozum gözlendi. Yirmiüç olguda (%60.5) pürifiye protein derives! (PPD) negatifliği belirlendi. Radyolojik evrelemeye göre; 1 olgu evre O (%2.5), 19 olgu evre 1 (%47.5), 14 olgu evre 2 (%35), 5 olgu evre 3 (%12.5), 1 olgu evre 4 (%2.5) olarak değerlendirildi. Radyolojik evre ile semptomlar ve PPD cevabı arasında ilişki yoktu. Otuzsekiz olgunun 15'inde FEV-, (%40) ve 30 olgunun 18 (%60)'inde DLCO %80'in altında idi. Hastalığın farklı evrelerinde FE\A| ve FVC değerlerinin her ikisi de farklılık gösterdi. Serum anjiyotensin konverting enzim yüksekliği 27 olgunun 9 (%33)'unda saptandı. Onyedi olgunun 9 (%53)'unda CD4/CD8 oranı 3.5'in üzerinde idi. Olguların 25'i en az 1 yıl izlendi. Radyolojik olarak olguların 15'inde gerileme, kalanında ise stabilite saptandı. Sonuç olarak, olgularımızın çoğunluğunu radyolojik olarak evre 1 ve 2 olgular oluşturmaktadır. Sarkoidoz olgularımızda bozulan başlıca solunum fonksiyon testi DLCO'dur. Bununla birlikte evre ile ilişkili olarak FEV1 ve FVC'de de değişiklik olmaktadır.
Abstract
The aim of the study was to evaluate the results of the clinical, laboratory, respiratory function test parameters including DLCO, and relation of them with radiological stage. In this retrospective study, 40 patients (male/female ratio: 12/28, mean age 40 ± 1 3 years) with sarcoidosis who diagnosed between 1996 to 2000 years, were included. The most com-mon symptom was cough (26 patients, 65%). Two patients had family history for sarcoidosis. Uveitis or chorioretinitis in 6 of the 27pts (22%) and erythema nodosum in 10 pts (25%) were observed. Twenty-three of the pts (60.5%) were un-responsive to purified protein derivative (PPD) skin testing. According to radiological staging, Stage 0 in 1 pt (2.5%), sta-ge 1 in 19 pts (47.5%), stage 2 in 14 pts (35%), stage 3 in 5 pts (12.5%) and stage 4 in 1 pts (2.5%) were detected. Symptoms and PPD response were not associated with radiological stage of the disease. Predicted value of FEV-] in 15 of the 38 pts (40%) and predicted value of DLCO in 18 of the 30 pts (60%) were lower than normal (80% of the predic-ted). Both FEV-] and FVC values were changing among the stages of disease. CD4/CD8 ratio was higher than 3.5 in 9 ofthe 17 pts (53%). Serum angiotensin converting enzyme levels were increased in 9 of the 27 pts (33%). Twenty-five of the pts have been followed-up at least one year. Radiological improvement was observed in 15 patients and remaining was stable. In conclusion, most of our patients were in stage 1 and 2. The mainly deteriorated respiratory function test parameter was DLCO. In addition, FEV-) and FVC values also changed in different stages of the disease.
